Objective: To estimate the consumption and expenses with the Specialized Pharmaceutical Assistance Component (CEAF) in Brazil. Methods: A time series study was carried out, based on data on consumption and expenditure on CEAF medicines in Brazil, from 2012 to 2021. Data from this study were extracted through the Ambulatory Information System of the System Health System (SIA/SUS) and the Information System on Public Health Budgets (SIOPS). Results: Important differences were identified in the accounting of CEAF drug expenditures by SIA/SUS and SIOPS, and SIA/SUS expenditures were underestimated in relation to those recorded in SIOPS. According to the SIA/SUS, the Southeast region had the highest expenditure on CEAF medicines, followed by the South, Northeast, Midwest and North regions. In addition, when checking the quantities of pharmaceutical units purchased, a gradual increase was observed over the years (2012 - 2021). Conclusion: Spending on CEAF drugs has remained stable over the years. On the other hand, it was observed that, over the years, there was an increase in the number of pharmaceutical units acquired, verifying a better application of public resources among the federal entities.
