Resumo: The objective of this research was to analyze the spatialization of malaria incidence in Manaus in the period from 2003 to 2022 and the relationship with urban socio-environmental vulnerability. Data from autochthonous cases (probable site of infection) were used and vulnerability indicators were selected according to a literature review. The median technique was applied to classify neighborhoods as low, medium-low, medium-high, and high vulnerability. The results showed that the spatial incidence of malaria predominated in the neighborhoods of Tarumã, Santa Etelvina, Puraquequara and Colônia Antonio Aleixo, neighborhoods that originated from irregular occupations and formed the limit between the expansion of the "urban" and "rural" fabric. The spatialization of malaria incidence advanced as urban occupations expanded territorially in the city. The mapping of socio-environmental vulnerability showed that the neighborhoods that presented high and medium-high vulnerability were the same ones that had a high incidence of malaria. The production of new urban spaces materialized by the occupations in Manaus is the very reproduction of malaria risks, as well as socio-spatial inequalities, since the places are composed of poorer social groups, who experience the precariousness of sanitation and health services, which influences the greater risk of contracting malaria.
