Objective: The aim of this study is to report the inferior first molar unitary rehabilitation in an individual, using an implant with a macro geometry proposal that favors greater primary stability and shortened osseointegration (Helix Grand Morse). This implant has a helical shape, having differences in its structure that are directly linked to its greater retention in any type of bone. Generating the clinical hypothesis that posterior cases with extracellular alveolus have good indications in immediate loading immediate fixation and precocious confection of definitive prosthesis. MATERIAL AND METHOD: Melanoderma patient, male, 31 years old, presented with the first lower left molar (36) treated endodontically, with diagnosis established by digital radiography of trephination of the distal root canal and coronary fracture. The element was condemned and subsequently performed minimally traumatic exodontia of the element, immediate installation of a Grand Morse implant and immediate provisional prosthesis. Periapical radiographs were performed immediately after surgery, at 30 days, 3 months and 6 months after implant installation. Results: After the conclusion of the case, we obtained the expected result of osseointegration as seen by radiographs, adaptation and rehabilitation of function and aesthetics. Conclusions: The clinical case presented showed that, after 6 months, favorable and aesthetic results are possible to achieve with the installation of a post-extraction alveolus implant and installation of immediate temporary body in the posterior region.
