Introduction: Macroglossia is the abnormal growth of the tongue, which presents a larger size than the buccal cavity allows. This alteration may be associated with dento-musculoskeletal deformities and instability of orthodontic-surgical treatment, as well as favoring the appearance of chewing, speech and breathing disorders. The procedure for surgical reduction of the tongue is atypical and is indicated for functional or aesthetic correction. Method: This paper presents a clinical case of a patient with relative macroglossia and dentofacial deformity with Angle Class III malocclusion diagnosed with obstructive sleep apnea syndrome. Result: Orthognathic surgery was performed to correct dentofacial deformity by advancement and maxillary expansion and mentoplasty using the basal horizontal osteotomy technique and partial glossectomy using the Egyedi-Obwegeser technique at the same surgical moment. Conclusion: The techniques used in this case were satisfactory for correction of changes, restoring function and aesthetics.
