Resumo: | Pain can be defined as an unpleasant sensory, emotional and subjective experience, considered essential for human survival. However, when it persists, it is referred to as chronic pain. It no longer fulfills this vital protection role and is considered a disabling condition, incurring serious physical, emotional, labor and social related damage. Among the syndromes whose set of symptoms revolves around chronic pain, fibromyalgia is considered one of the most common clinical rheumatologic conditions, recognized as a global chronic health problem and requiring excessive use of healthcare services. Given the limited effectiveness of the pharmacological approach in treating fibromyalgia, alternative interventions have been proposed in recent years, including mindfulness-based interventions, which have been increasingly gaining attention from health treatment programs. Basically, mindfulness can be defined as an awareness of the present moment of everything that arises internally or externally, without making judgments, as opposed to having the mind wander or seek to actively solve problems. In this sense, through a systematic literature review, the present investigation aimed to evaluate the effectiveness of mindfulness-based interventions in pain perception and attention modulation in patients diagnosed with fibromyalgia. The research strategy was carried out by searching the Medline, Embase, PsycINFO and Web of Science databases. The descriptors used included the terms: “mindfulness” and “fibromyalgia.” Twelve articles were selected at the end of the survey. Their results pointed towards the effectiveness of mindfulness-based interventions, considered a complementary resource to conventional pharmacological treatment in patients with fibromyalgia. The benefits of mindfulness practice were identified, mainly related to the quality of life, social engagement, and physical and psychological well-being. Regarding pain management, positive changes associated with the ability to deal with pain were observed. As for the attentive bias, an efficient disengagement related to the threat of pain was observed. Concerning psychological aspects, mindfulness-based programs showed significant mental health effects, mainly involving symptoms of depression, anxiety, and perceived stress. As for social aspects, mindfulness resulted in positive outcomes related to a greater sense of social responsibility and engagement in citizenship issues. These findings suggest that mindfulness techniques can positively impact the perception of well-being in patients suffering from chronic fibromyalgia pain. Furthermore, it emphasizes the importance of deepening our understanding of the theme in question through future research. |

Porém, quando ela persiste, passa a ser referida como dor crônica, e não mais atende a esse papel de proteção vital, sendo, então, considerada uma condição incapacitante, incorrendo em sérios prejuízos físicos, emocionais, laborais e sociais. Dentre o grupo de síndromes cujo conjunto de sintomas gira em torno da dor crônica, a fibromialgia destaca-se por ser considerada uma das condições clínicas reumatológicas mais comuns, reconhecida como um problema de saúde crônico global, e que requer o uso excessivo dos serviços de saúde. Diante da eficácia limitada da abordagem farmacológica no tratamento da fibromialgia, intervenções alternativas têm sido propostas nos últimos anos, dentre elas as intervenções baseadas no Mindfulness, as quais têm ganhado atenção crescente de programas de tratamento em saúde. Basicamente, o mindfulness pode ser definido como a consciência no momento presente a tudo que surge interna ou externamente, sem a realização de julgamentos, em oposição à divagação da mente ou à busca pela solução ativa de problemas. Nesse sentido, a presente investigação teve como objetivo avaliar, por meio de revisão sistemática da literatura, a eficácia das intervenções baseadas em mindfulness na percepção da dor e na modulação atentiva em pacientes diagnosticados com fibromialgia. A estratégia de busca ocorreu por meio de pesquisa nas bases Medline, Embase, PsycINFO e Web of Science. Os descritores utilizados incluíram os termos: “mindfulness” e “fibromyalgia”. Foram selecionados 12 artigos ao final do levantamento realizado, e seus resultados apontaram em direção à efetividade das intervenções baseadas em mindfulness, consideradas como um recurso complementar ao tratamento farmacológico convencional em pacientes com fibromialgia. Identificou-se benefícios da prática do mindfulness relacionados principalmente à qualidade de vida, ao engajamento social, e ao bem-estar físico e psicológico. Com relação ao manejo da dor, observou-se mudanças positivas associadas à capacidade em lidar com a dor. Quanto ao viés atentivo, notou-se um desengajamento eficiente relacionado à ameaça de dor. No que se refere aos aspectos psicológicos, os programas baseados no mindfulness apresentaram eficácia significativa relacionada à saúde mental de modo geral, envolvendo principalmente sintomas de depressão, ansiedade e estresse percebido. Quanto aos aspectos sociais, a prática do mindfulness resultou em desfechos positivos relacionados ao maior senso de responsabilidade social e engajamento em questões de cidadania. Estes achados sugerem que a prática das técnicas de mindfulness pode contribuir positivamente para uma melhora na percepção de bem-estar em pacientes que sofrem com dor crônica na fibromialgia.
