Abordagem multidisciplinar na reabilitação oral de paciente idoso: relato de caso

Resumo: | The care of the elderly requires attention to their physical, mental and social health, for an effective promotion of their health. The present study reports the treatment of an edentulous patient involving dentistry, medicine and psychology. A 63-year-old male patient attended the overdenture extension project for total edentulous in geriatrics complaining of unsatisfactory aesthetics, instability of his dentures, especially the mandibular, and difficulty in eating. In the anamnesis and in the satisfaction, skill (VAS scale) and quality of life (OHIP EDENT) questionnaires, the patient reported high rates of dissatisfaction with current appliances, chewing difficulty, poor quality of life and social reclusion. Clinical examination identified a pair of mucosally supported dentures with insufficient retention, occlusal instability, overextension of the bases, and teeth colouring that displeased the patient. New maxillar complete denture and mandibular implant-suported overdenture were planned. The same was referred for geriatric and psychological medical care. Medical examinations showed systemic changes that contraindicated implant placement immediately, but no changes in cognitive and motor patterns were identified (mini-mental test, Lawton scale, and Katz scale). Therefore, bimaxillary complete dentures were made until release to surgical procedure. The questionnaires were reapplied after the installation of new dental prosthesis and the results showed a dramatic improvement in satisfaction and chewing ability indices, in quality of life and social reinclusion. The follow-up with the psychologist showed availability and satisfaction with and with the treatment and the patient reported feeling welcomed by the entire medical and dental team, as well as that these care improved their quality of life. Therefore, teamwork aimed at this population is extremely important to ensure the best planning and guarantee the health promotion in its entirety. Keywords: overdenture, implant rehabilitation, quality of life.Trabalho de Conclusão de Curso (Graduação)O atendimento de idosos exige atenção ao seu estado de saúde físico, psíquico e social, para que assim possa ser feita uma efetiva promoção à sua saúde. Este trabalho relata o tratamento de paciente desdentado total envolvendo odontologia, medicina e psicologia. Paciente do gênero masculino, 63 anos, compareceu ao projeto de extensão overdenture para desdentados totais na geriatria queixando-se de estética insatisfatória, instabilidade de suas próteses, principalmente a inferior e dificuldade de alimentar-se. Na anamnese e nos questionários de satisfação, habilidade (escala EVA) e qualidade de vida (OHIP EDENT), o paciente relatou altos índices de insatisfação com os aparelhos atuais, dificuldade mastigatória, pobre qualidade de vida e reclusão social. O exame clínico identificou um par de próteses mucosuportadas apresentando insuficiente retenção, instabilidade oclusal, sobreextensão das bases e coloração dos dentes que desagradavam o paciente. Foi planejada nova prótese total superior e overdenture inferior sobre implantes. O mesmo foi encaminhado para atendimento médico geriátrico e psicológico. Os exames médicos apontaram alterações sistêmicas que contraindicaram a instalação dos implantes imediatamente, mas nenhuma alteração dos padrões cognitivos e motores foi identificada (teste mini-mental, escala de Lawton e escala de Katz). Assim sendo, foram feitas próteses totais bimaxilares até liberação para procedimento cirúrgico. Os questionários foram reaplicados após a instalação das novas próteses e os resultados mostraram uma melhora vertiginosa dos índices de satisfação e habilidade mastigatória, na qualidade de vida e reinclusão social. O acompanhamento com a psicóloga mostrou disponibilidade e satisfação para e com o tratamento e o paciente relatou se sentir acolhido por toda a equipe médica e odontológica, bem como que esses cuidados melhoraram sua qualidade de vida. Portanto, o trabalho em equipe destinado a essa população é de suma importância para garantir o melhor planejamento e garantir a promoção de saúde na sua integralidade.
