The anterior open bite is a challenging malocclusion. It is a condition of complex etiology that manifests itself in several facial patterns, occlusal arrangements and developmental phases. This work aims to clarify the treatment of anterior open bite emphasizing the surgical modalities, since they are traditional, effective and stable and illustrating with a case showing the occlusal, functional and esthetic benefits of the surgical treatment. The treatment flowchart of this condition involves a profound study for correct diagnosis and efficient treatment differently, either prior to pubertal outbreak, in adults with facial deformity or in adults without facial deformity. Complexity, morbidity and high cost are factors that hinder acceptance to Combined orthodontic and surgical treatment favoring the recent use of temporary skeletal anchorage devices. Nonetheless, the literature emphasizes that there is a weak scientific evidence regarding the case solutioning by temporary anchorage devices and converges to the criticism of occlusal treatment with no considering the function and aesthetic aspects of the face.
