Cisto odontogênico glandular: relato de caso

Resumo: Glandular Odontogenic Cyst (GCO) is a rare benign odontogenic lesion that was first described in 1987. It is defined as a cyst arising in the areas of eruption of teeth in both arches, characterized by an epithelium formed by cuboid or columnar cells, both on the surface and crypts within the thickness of the epithelium.It occurs more frequently in the anterior region of the mandible, in adults over 40 years of age, predisposed by the male gender. It tends to recur, has slow growth and appears as an asymptomatic swelling of the jaw. This report presents a case of a male patient, 58 years old, who was referred to the University after attending a private practice for rehabilitation with implant prosthesis.The lesion was discovered in the panoramic radiography for implant planning. An intraoral clinical examination revealed right mandibular swelling.In panoramic and cone beam computed tomography images, a radiolucent, unilocular and well definedlesion was observed, extending from the midline region to the first right lower first molar. Incisional biopsy and marsupialization of the lesion were performed, obtaining the diagnosis of glandular odontogenic cyst. After five months, enucleation and curettage of the lesion were performed. Three months after enucleation, a new CBCT scan was performed and it could be observed new bone formation in the region, with no evidence of recurrence. The patient is followed up every six months in the oral diagnosis clinic.

É definido como um cisto que surge nas áreas de erupção dos dentes em ambas arcadas, caracterizado por um epitélio formado por células cuboides ou colunares, ambas na superfície e criptas dentro da espessura do epitélio. Surge com maior frequência na região anterior da mandíbula, em adultos acima de 40 anos, com predisposição pelo gênero masculino. Tende a recorrer, possui crescimento lento e aparecem como um aumento de volume assintomático. Este trabalho relata o caso de um paciente do sexo masculino, 58 anos de idade, que foi encaminhado à UFU após ter procurado clínica particular para reabilitação com prótese sobre implante. A lesão foi descoberta no exame radiográfico panorâmico para planejamento dos implantes. Ao exame clínico intraoral observou-se aumento de volume mandibular do lado direito. Nas imagens panorâmica e tomográfica de feixe cônico visualizava-se uma lesão radiolúcida, unilocular e bem delimitada, se estendendo da região de sínfise ao primeiro molar inferior direito. Foi realizada biópsia incisional e marsupilização da lesão, obtendo-se o diagnóstico final de cisto odontogênico glandular. Depois de cinco meses foi realizada a enucleação e curetagem da lesão. Realizou-se nova tomografia computadorizada três meses após a enucleação e observou-se neoformação óssea na região, sem indícios de recidiva da lesão.
