Consumo alimentar de macro e micro nutrientes pós-colostomia por câncer colorretal: estudo prospectivo.

Resumo: | Introduction: Colorectal cancer is the third type of cancer with the highest number of deaths in Brazil. Due to symptoms it is common to change food intake and unintentional weight loss negatively impacting nutritional status. In addition, treatment involves colostomy surgery which because of gastrointestinal manifestations, leads to important dietary restrictions in this patient. The aim of this study was to evaluate the consumption of macro and micronutrients after ostomy. Methods: 28 individuals who underwent colostomy surgery participated in this prospective study. Dietary intake data was obtained from 24-h dietary recalls and used to analyze food consumption and the probability and prevalence of inadequacy of macro and micronutrients, after surgery (<2 months; T0) and 6 months later (T1). Results: The sample consisted of 57.2% elderly and 64.3% women. After 6-8 months of colostomy there was a decrease in the consumption of carbohydrate, polyunsaturated fat, vit.B6 and magnesium and an increase in the values of protein, phosphorus and calcium. The percentage of inadequacy increased over time for energy, carbohydrate and lipid. There was also an increase or maintenance of the high prevalence of inadequacy over time for several micronutrients such as calcium, vit.B6, riboflavin (men) and fibers (women) ranging from 79% to 100%. Conclusion: in the evaluated population colostomized patients after 6 months had lower consumption of some nutrients and a percentage of inadequacy of macronutrients as well as a high prevalence of inadequacy of some micronutrients. The adequacy of nutrients is important to avoid future clinical complications. |

Devido aos sintomas é comum a alteração da ingestão alimentar e perda de peso não intencional, impactando negativamente no estado nutricional. Além disso, o tratamento envolve a cirurgia de colostomia, que por causa das manifestações gastrintestinais leva a restrições alimentares importantes neste paciente. O objetivo deste estudo foi avaliar o consumo de macro e micronutrientes pós-ostomia. Métodos: Participaram deste estudo prospectivo 28 indivíduos com câncer colorretal submetidos a cirurgia de colostomia. Foram analisados, por meio de recordatório alimentar 24h, o consumo alimentar de macro/micronutrientes e a probabilidade e prevalência de inadequação alimentar, após a realização da cirurgia (<2 meses; T0) e 6 meses depois no T1. Resultados: A amostra foi composta por 57,2% de idosos e 64,3% de mulheres. Após 6-8 meses de colostomia, foi observado uma diminuição no consumo de carboidrato, gordura poli-insaturada, vit.B6 e magnésio e um aumento nos valores de proteína, fósforo e cálcio. O percentual de inadequação aumentou no tempo para energia, carboidrato e lipídio. Foi observado ainda, aumento ou manutenção da elevada prevalência de inadequação no tempo para diversos micronutrientes como cálcio, vit.B6, riboflavina (homens) e fibras (mulheres) variando de 79% a 100%. Conclusão: Após 6 meses, na população avaliada, pacientes colostomizados apresentaram menor consumo de alguns nutrientes e um percentual de inadequação de macronutrientes, bem como uma elevada prevalência de inadequação de alguns micronutrientes.
