Análise funcional preliminar do tratamento cirúrgico das fraturas condilares: estudo de coorte retrospectivo

Resumo: In order to understand the fractures of the neck and to construct the database with the appropriate frequencies to the treatments, the main objective is the obtention of the longitudinal clinical criteria after the clinical treatment of condylar fractures. The insertion criteria were the emergent adults with a diagnosis of colon fracture, which were completed in 2010, in 2018. Volunteers who participate in fracture of the mandibular angle, or fractures of other regions, are not related to the condylar area and to those who did not complete the treatment, were excluded. The investigation of the anamnesis and the clinical examination was used the questionnaire Diagnostic Criteria for Research in Temporomandibular Dysfunction (RDC / TMD). The presence of symptomatology was evaluated through the Visual Analogue Scale - EVA. The study was performed in the clinical examination of a maxillary, protuscular and right lateral surgery. The highest prevalence of mandibular condyle fractures was found with no male sex, with a predominance of 75%. The location of the most affected fracture was a subcondylar region. In moving mandibular as opening, protrusion and good direction in most patients in their normal state. The most widely used oral opening pattern was the straight pattern. Pain was absent in most patients, as was a TMD. It concludes that the studies with greater number of patients and comparing the different types of treatment for conditional fractures are mandatory and important to confirm the findings.

Os critérios de inclusão foram indivíduos adultos com diagnóstico de fratura de côndilo, que concluíram o tratamento cirurgicamente no período de 2010 a 2018. Os voluntários que apresentaram apenas fratura de ângulo mandibular, ou fraturas de outras regiões que não envolveram a área condilar e aqueles que não finalizaram o tratamento, foram excluídos. Para realização da anamnese e exame clínico foi utilizado o questionário Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). A presença de sintomatologia foi avaliada por meio da Escala Visual Analógica – EVA. Foi avaliado no exame clinico a abertura bucal máxima, protusão e os movimentos de excursão lateral direita e esquerda. A maior prevalência de fraturas de côndilo mandibular foi encontrada no sexo masculino, com uma predominância de 75%. A localização da fratura de maior acometimento foi a região subcondilar. Os movimentos mandibulares como abertura, protusão e excursões apresentaram-se na maioria dos pacientes em seu estado de normalidade. O padrão de abertura bucal de maior prevalência foi o padrão reto. A dor estava ausente na maioria dos pacientes, assim como a DTM.
