Origem, diagnóstico, prevenção e considerações sobre o tratamento da síndrome da combinação: revisão de literatura e relato de caso

The Combination Syndrome or Kelly’s Syndrome was initially described by Ellsworth Kelly, in 1972, and was related to the condition of edentulism in which a totally edentulous maxilla and a Kennedy class I mandible are observed, with teeth remaining only in the anterior sextant or, more contemporarily, by an implant prostheses situation in which support is provided by implants only in the mandibular anterior sextant. It is clinically characterized by marked bone loss in the anterior region of the maxilla, bone loss in the posterior region of the mandible, papillomatous hyperplasia of the palatal mucosa, extrusion of the mandibular lower teeth and fibrous hyperplasia of the maxillary tuberosity. Due to its peculiarity, several patients have their rehabilitation conducted incorrectly, allowing the maintenance of the signs that contribute to the accelerated evolution of the syndrome. This work consists of a literature review on the subject, in addition to a clinical case report of a patient with the combination syndrome, to disseminate and contribute to a better rehabilitation intervention for the individual who is in this situation, offering more health and comfort. Therefore, it is concluded that the dentist must be able to clinically recognize the characteristics of the combination syndrome and conduct rehabilitation to avoid its clinical signs and restore the normality of the stomatognathic system, regardless of the chosen treatment modality.
