Análise tridimensional do volume da via aérea superior e da posição da coluna cervical após cirurgia ortognática com avanço e rotação anti-horária do complexo maxilo-mandibular

Resumo: The present study aimed to evaluate the alteration of the airway volume and cervical spine position of patients submitted to bimaxilary orthognathic surgery with counterclockwise rotation of the occlusal plane. Cone beam computed tomography (CBCT) scans of 44 patients, facial pattern II who underwent orthognathic surgery with advancement and counterclockwise rotation of maxillo-mandibular complex were evaluated. Airway and cephalometrics measurements were obtained before surgery (T1), immediately after surgery (T2) and at least 09 months after surgery (T3) using Dolphin Imaging® 11.8. The airway parameters, such as area, minimum axial area (MAA) and volume (AIW) were significantly increased after surgery (P ≤0.01). There was also a significant difference in head posture on post-surgical follow-up (T3-T1), based on changes of OPT.SN angle (mean value 2.9°) and of CVT.SN angle (mean value 3.5°). Significant correlations between the increased of upper airway, surgical movements and changes in head posture were found (P ≤0.05). Therefore, according to these results, orthognatic surgery with advancement and counterclockwise rotation of maxillomandibular complex can increase the upper airway dimensions and improve the head posture.

Foram avaliadas Tomografias Computadorizadas Cone Beam (TCCB) de 44 pacientes que apresentavam padrão facial classe II/ face longa que foram submetidos a este tratamento cirúrgico. Através do software Dolphin Imaging® 11.8 os parâmetros de via aérea superior e posterior, bem como as medidas cefalométricas foram obtidos antes da cirurgia (T1), imediatamente após a cirurgia (T2) e pelo menos 09 meses após a cirurgia (T3). Os paramêtros avaliados da via aérea superior, como área total, mínima área axial (MAA), e volume (AIW) tiverem um aumento estatisticamente significante após a cirurgia (P ≤0.01). Houve também uma alteração significante da posição da cabeça no pós-operatório tardio (T3-T1), baseado nas alterações dos ângulos OPT.SN (média 2.9º) e do ângulo CVT.SN (média de 3.5º). Correlações estatisticamente significantes foram obtidas entre o aumento da via aérea superior posterior, os movimentos cirúrgicos e entre as mudanças da posição da cabeça (P ≤0.05).
