Reabilitação mandibular por meio da overdenture com sistema de retenção barra-clipe: relato de caso

Edentulism is a condition that compromises oral health and is a determining factor in the quality of life. Considering the number of completely edentulous patients, various types of treatment may be indicated, including conventional complete dentures and implant-retained or tooth- and mucosa-supported overdentures. Preserving the supporting teeth used as pillars for overdentures favors prosthetic treatment by improving biomechanical principles and reducing alveolar bone resorption. In this context, the present case report aims to describe the rehabilitation through an upper conventional complete denture associated with an overdenture with a bar-clip retention system on the lower arch. A 64-year-old male patient sought the Dental Hospital of the Federal University of Uberlândia complaining of a lack of retention in his complete dentures. During the clinical examination, wear of the artificial teeth and the absence of retention and stability of the lower prosthesis were observed. The remaining teeth (33 and 43) had satisfactory endodontic treatment and the presence of a cemented metal bar. Based on this information, the proposal was to make a new upper mucosa-supported complete denture and an inferior overdenture using the bar-clip retention mechanism. The clinical sequence adopted included: anatomical molding, master molding, intermaxillary registration, and tooth selection; esthetic and functional trial of the artificial teeth and gum color selection; installation and attachment of the clip onto the bar. It is concluded that the overdenture is an effective option in rehabilitating edentulous patients, demonstrating notable benefits in terms of patient satisfaction. Additionally, biomechanical principles were optimized, restoring comfort with the use of the prosthetic appliance.
