Infecções hospitalares em pacientes idosos: aspectos epidemiológicos clássicos e moleculares associados a Staphylococcus aureus e Enterococcus spp

Resumo: Three one-week survey was conducted to determine the prevalence rate of nosocomial infections in elderly patients (65 years of age or older). We examined nosocomial and community infections rates and their relationship with intrinsic and extrinsic risk factors in a Brazilian University Hospital. Additionally, a colonization survey was carried out from their nares, oropharynx and intestine the presence of Staphylococcus aureus and Enterococcus spp. Finally, a laboratory surveillance was conducted during the period to detected nosocomial bacteremias by this organisms considering adults patients and elderly patients. Isolates were identified to species levei by classical methods and antimicrobial susceptibility by using gel diffusion and diluition methods. mecA gene and van genes were investigate in MRSA isolates (“Methicillin- resistant Staphylococcus aureus”) and Enterococcus spp., respectively, by polymerase chain reaction. The pulsed-fíeld gel electrophoresis (PFGE) pattems to eleven Enterococcus isolates recovered in different clinics/services was performed. The overall prevalence of elderly patients with nosocomial and community infections were 16,1% and 25,6%, respectively without dififerences when compared with adults patients. On the other hand, when the elderly group was considered almost all risk factors (use of antibiotics, invasive devices, surgery and time o f hospitalization) were found to be significantly associated with hospital infection. The colonization rate for Staphylococcus aureus was high (55,6% in nostril and 38,9% in intestine), with 39% o f the isolates oxacillin resistant. Risk factors to nosocomial bacteremias by MRSA included: more than one diagnostic, surgery, time o f hospitalization, use o f antibiotics and devices invasives, but was not related with advanced age (60 years of age or older). We describe the first isolate of vancomycin-resistant Enterococcus faecalis (vanA) in a surgical patient, poly-traumatic, vancomycin and 3thcephalosporins treated. The strain was clonally related as shown by PFGE to another isolate who was not the same ward and was treated of different time.

Foram avaliadas as taxas de infecções hospitalares e comunitárias, principais síndromes infecciosas e os fatores de risco intrínsecos e extrínsecos relacionados, em pacientes adultos e idosos. Adicionalmente, foram realizadas coletas de materiais clínicos provenientes da narina, boca e cavidade anal para a pesquisa de Staphylococcus aureus e Enterococcus spp. Foi realizada uma vigilância no laboratório de Microbiologia do Hospital de Clínicas da Universidade Federal de Uberlândia (HC-UFU), de bacteremias hospitalares por esses microrganismos em pacientes adultos, incluindo os idosos; os isolados foram identificados em nível de gênero e espécies por testes tradicionais e a resistência “/« vitro" aos antimicrobianos foi determinada através dos testes de difusão e diluição em gel. Foi detectada ainda a presença do gene mecA. e genes van em amostras de MRSA (“Methicillin-resistant Staphylococcus aureus") e enterococos, respectivamente, através da técnica multiplex PCR. Os graus de similaridade genética entre amostras de Enterococcus selecionadas, considerando-se as clínicas/serviços mais representativas foram analisadas através do PFGE (“Pulsed-field Gel Electrophoresis”). As taxas de infecções hospitalares e comunitárias foram 16,1% e 25,6%, respectivamente, sem diferenças estatísticas entre adultos e idosos. Quando a análise estatística incluiu apenas os idosos, praticamente todos os fatores de risco considerados foram associados com infecção hospitalar, com destaque para: tempo de hospitalização longo, em uso de antibiótico e de mais de dois procedimentos invasivos sobressaindo cateter intravascular e cirurgia. Os pacientes idosos apresentaram uma alta taxa de colonização por Staphylococcus aureus (55,6% na narina e 38,9% no intestino), com 39,0% de resistência à oxacilina. A análise univariada dos fatores de risco para bacteremias hospitalares por MRSA indicou significância para osseguintes: presença de mais de um diagnóstico, paciente cirúrgico, tempo de internação, uso de antibióticos e procedimentos invasivos como sonda vesical e dreno, sem no entanto relação de risco com os pacientes com mais de 60 anos. É descrito o primeiro isolado de Enterococcus faecalis (vanA) em uma paciente com episódio de infecção cirúrgica, politraumatizada, em uso de vancomicina e cefalosporina de 3a geração. Esta amostra apresentou um perfil de similaridade de aproximadamente 80,0% com um segundo isolado, sem no entanto, qualquer relação epidemiológica quanto ao tempo e clínica de internação.
