Resumo: In this transversal epidemiologic study were evaluated the nutritional status, prevalence of anemia and associated factors to hemoglobin level of 155 elderly individuals registered in the Programa de Saúde da Família (PSF) in the city of Viçosa, Minas Gerais. Blood samples of 8 ml were collected by vein puncture and used to determine hemoglobin concentrations and hematimetric indices by automated electronic counting, seric ferritin by turbidimetry and C- reactive protein by latex- agglutination method. The measures of weight, height, waist circumference (WC) and hip circumference were checked and Body Mass Index (BMI) and Waist-to-Hip Ratio (WHR) were calculated. A questionnaire for the socioeconomic and health conditions characterization and a food frequency questionnaire for foods sources of iron and vitamin C were applied. The limit values to classify the nutritional status of iron were: male - hemoglobin <13 g/dL, hematocrit <39%, MCV <80 fL, ferritin <15 g/L; female - hemoglobin <12 g/dL, hematocrit <36%, MCV <80 fL, ferritin <15 g/L. Anemia was considered using hemoglobin concentration, iron deficiency using seric ferritin concentration and irondeficiency anemia when hemoglobin and ferritin or MCV were below the cut points. The cut points for classification of BMI were <22 kg/m2 for lowweight and BMI >27 kg/m2 for the overweight. The cut points for WC and WHR were: WC> 94cm and WHR> 1.0 (male), WC >80cm and WHR >0.85 (female). It was observed association between socioeconomic and health factors and nutritional status; as well as association of socioeconomic and health factors and nutritional status with the hemoglobin level of elderly. High prevalence of low weight (16.6%) and overweight (41.1%) were found, as well as values of WC (59.3%) and WHR (57.2%) above recommended. Females are more exposed to the related risks of obesity and their complications. Elderly with better socioeconomic situation presented larger values of WC and WHR and larger overweight frequencies. Sedentary elderly and with chronic morbidities presented larger values of Body Mass Index (BMI), WC and WHR. Dentate elderly presented larger weight measures, BMI and WC. Among the evaluated elderly, 4.5% (n=7) was diagnosed with anemia, and one of them (0.6%) with iron-deficiency anemia. Three elderly individuals(2.3%) were diagnosed with iron deficiency. There was found association among smaller hemoglobin levels (Hb) and low frequency of chicken and foods enriched with iron consumption. Females were associated with smallest hemoglobin levels, as well as the professional work at the largest level. Elderly with smaller levels of Hb presented smaller weight, height and WHR; as well as larger number of chronic diseases and medicines consumed by day. In a general way, we conclude that the elderly individuals registered in PSF of Viçosa - MG presents high prevalence of inadequate nutritional status. However, anemia was not considered a public health problem among this elderly. The inadequate nutritional status and low hemoglobin levels were associated to the gender, socioeconomic and health conditions of elderly individuals.

Alíquotas de 8 mL de sangue coletadas por punção venosa foram utilizadas para determinar concentrações de hemoglobina e índices hematimétricos por contagem eletrônica automatizada, ferritina sérica por turbidimetria e Proteína C Reativa pelo método de aglutinação em látex. Foram aferidas as medidas de peso, estatura, circunferência da cintura (CC) e circunferência do quadril e calculados o Índice de Massa Corporal (IMC) e Razão Cintura/Quadril (RCQ). Foram aplicados dois questionários, o primeiro para a caracterização socioeconômica e condições de saúde e o segundo questionário de freqüência de consumo alimentar seletivo para os alimentos fontes de ferro e vitamina C. Os valores limítrofes para classificar o estado nutricional de ferro foram: sexo masculino - hemoglobina <13 g/dL, hematócrito <39%, VCM < 80 fL, ferritina < 15 &#956;g/L; sexo feminino hemoglobina <12 g/dL, hematócrito < 36%, VCM < 80 fL, ferritina < 15 &#956;g/L. Considerou-se anemia pela concentração de hemoglobina, deficiência de ferro pela concentração de ferritina sérica e anemia ferropriva quando hemoglobina e ferritina ou VCM estivessem abaixo dos pontos de corte. Os pontos de corte para classificação do IMC foram <22 kg/m2 para o baixo peso e IMC >27 kg/m2 para o sobrepeso. Os pontos de corte para CC e RCQ foram: CC > 94cm e RCQ > 1,0 (sexo masculino), CC>80 cm e RCQ >0,85 (sexo feminino). Verificou-se a associação de fatores socioeconômicos e de saúde ao estado nutricional; bem como a associação de fatores socioeconômicos, de saúde e estado nutricional ao nível de hemoglobina dos idosos. Foram encontradas altas prevalências de baixo peso (16,6%) e sobrepeso (41,1%), bem como de valores de CC (59,3%) e RCQ (57,2%) acima do recomendado. O sexo feminino se mostrou mais exposto aos riscos relacionados à obesidade e suas complicações. Idosos com maior nível socioeconômico apresentaram maiores valores de CC e RCQ e maiores freqüências de sobrepeso. Idosos sedentários e com morbidades crônicas apresentaram maiores valores de Índice de Massa Corporal (IMC), CC e RCQ. Idosos dentados apresentaram maiores medidas de peso e IMC e CC. Entre os idosos avaliados, 4,5% (n=7) foi diagnosticado com anemia, dos quais um idoso (0,6%) com anemia ferropriva. Três idosos (2,3%) foram diagnosticados com deficiência de ferro. Houve associação entre menores níveis de hemoglobina (Hb) e baixa freqüência de consumo de frango e alimentos enriquecidos com ferro. O sexo feminino foi associado a menores níveis de hemoglobina, bem como o trabalho profissional ao maior nível. Idosos com menores níveis de Hb apresentaram menor peso, estatura e RCQ; bem como maior número de doenças crônicas e de medicamentos consumidos por dia. De forma geral, conclui-se que os idosos cadastrados no PSF de Viçosa MG apresentam elevadas prevalências de inadequado estado nutricional. A anemia, no entanto, não foi considerada um problema de saúde pública entre idosos estudados.
