Leishmaniose tegumentar americana no município de Ilhéus- BA: Caracterização de casos humanos e fatores de risco associados

Resumo: | In Brazil, the American Cutaneous Leishmaniasis (ACL) is highly endemic, and has been a serious public health problem. The country currently confronts the spreading and urbanization, in big and middle-sized cities, of the ACL, which is primally zoonotic and related to forest environments. The present work valued the epidemiologic aspects of ACL in Ilhéus, and the possible risk factors associated to its appearance in the urban area of this city, with previous case reports notified as autochthones of this area. A revision of all investigation cards of ACL cases in Ilhéus, registered by the SVE/SMS/Ilhéus from January 2000 to December 2006 was performed, analyzing the follows information about the referred cases: age, sex, clinical forms, diagnostic date and illness evolution. A study on phlebotomine sand fly occurrence in the urban area of Ilhéus was carried out from March to August 2006, using CDC light traps. The canine infection was investigated through indirect fluorescent antibody test (IFAT) and BIOGENE enzyme- linked immunosorbent antibody test (ELISA) for calazar canine. Additionally, with the purpose of identifying risk factors for cutaneous leishmaniasis transmission in the urban area of Ilhéus, a matched case-control study was carried out involving the human population. The information about the human cases, sand fly fauna and dogs was analyzed with frequency tables. The human cases were also analyzed, with correlation studies between the probable infection date and climate data. The possible risk factors were determinated with X² MacNemar calculation, Odds Ration (OR) and 95% Confidence Intervals (CI). Analysis of investigation records showed that ACL affects mainly adult subjects, with cutaneous lesions as the principal clinical form. The seasonality analysis showed the probable infection period to be quarter September-November (Spring). No statistically significant correlation was observed (p< 0.05) between case number and climate data (rain fall, humidity and temperature). On the other hands, periods with high rainfall and humidity showed a smaller number of cases. Lu. Cortelezzii was the one species collected in urban area. Three seropositive dogs were identified by the immunofluorescence assay. The case-control study showed that the age group 13 to 44 years (p= 0.0061; OR=0.13; 95% CI= 0.076-0.22), living close to woodlands, was associated with decreased risk; the habit of going to the rural area of Ilhéus showed to be a risk factor to ACL (p= 0.003; OR= 5.5; 95%; CI= 1.75- 17.29). With these results, we can conclude that ACL in Ilhéus is not urban, since no typical vector species were found, and the human cases were associated to the habit of going to the rural area. Additionally, the case profiles, adult males, showed that the subjects exposed demonstrate non- compatible characteristics with a wider-ranging exposure, as should be expected to happen if the illness transmission cycle took place in Ilhéus urban area. We also note the importance of maintaining a strict registry of human cases, with maximum rigidity, to guarantee specific diagnostic for ACL, making possible epidemiological analysis more rustworthy. |

Primariamente zoonótica, relacionada a ambientes silvestres, atualmente o país enfrenta a expansão e urbanização da LTA em cidades de médio e grande porte. O presente trabalho avaliou os aspectos epidemiológicos da LTA no município de Ilhéus-BA e os possíveis fatores de risco associados ao seu aparecimento na área urbana da cidade, com registros recentes de casos notificados como autóctones dessa área. Foi realizada revisão de todas as fichas de investigação dos casos de LTA procedentes do município de Ilhéus, notificados pelo SVE/SMS/Ilhéus durante o período de janeiro de 2000 a dezembro de 2006, obtendo-se as seguintes informações referentes aos casos: idade, sexo, forma clínica, data de diagnóstico e evolução da doença. Também foi realizado estudo sobre a fauna flebotomínea na área urbana, durante os meses de março a agosto de 2006, utilizando armadilhas luminosas tipo CDC (Center of Disease Control), além da coleta transversal de amostras de soro sangüíneo, para pesquisa de anticorpos (RIFI e ELISA) e dados referentes a cães domiciliados na área urbana e rural. Adicionalmente, foi realizado estudo caso-controle pareado envolvendo a população humana residente na área urbana para identificação de fatores de risco associados. Os dados relativos aos casos humanos, fauna flebotomínea e os cães foram analisados a partir da construção de tabelas de freqüência; os casos huamnos também foram analisados a partir de estudos de correlação entre a provável data de infecção e dados climatológicos (pluviosidade, umidade e temperatura). Os possíveis fatores de risco foram determinados a partir do cálculo do χ2 de MacNemar, da razão de chances e respectivo intervalo com 95% de confiança. De acordo com a análise das fichas de investigação, a LTA acomete, principalmente, indivíduos adultos, em idade economicamente ativa, com a forma clínica cutânea como predominante. A análise da sazonalidade demonstrou ser o trimestre setembro-novembro (primavera) o provável período de infecção. Não foram identificadas correlações significativas (p < 0,05) entre o número de casos e dados climatológicos, entretanto, os períodos de pluviosidade e umidade relativa altas, concentraram o menor número de casos. As coletas entomológicas indicam ser a espécie Lu. cortelezzii o único componente da fauna flebotomínea na área urbana. Dos 381 cães pesquisados, apresentaram títulos positivos para Leishmania spp., através da RIFI, apenas três animais. O estudo caso-controle revelou que a faixa etária 13 a 44 anos (p = 0,0061; RC = 0,13; IC 95% = 0,076- 0,22) e morar próximo à mata (p = 0,005; RC = 0,09; IC 95% = 0,05- 0,16) constituíram fatores de proteção para a doença e o hábito de freqüentar a área rural do município, fator de risco para LTA (p = 0,003; RC = 5,5; IC 95% = 1,75- 17,29). A partir dos resultados encontrados podemos concluir que, aparentemente, a LTA no município de Ilhéus, não tenha passado por processo de urbanização, uma vez que não foram identificados vetores típicos do agente e os casos humanos terem estado associados ao fato do paciente freqüentar a área rural. Adicionalmente, o perfil dos casos, homens em idade economicamente ativa, revela que os indivíduos expostos apresentam características não compatíveis com uma exposição mais abrangente, como se esperaria acontecer se a doença tivesse seu ciclo de transmissão instalado na área urbana.
