Fatores associados ao estado emocional materno no período pós-parto e sua relação com a prática da amamentação

Resumo: The aim of the study were to determine the factors associated with maternal postpartum emotional state and to evaluate its relationship with breastfeeding, in Viçosa (MG), Brazil. It is cross-sectional study of 168 postpartum women from the Viçosa Polyclinic Municipal s sector immunization. We held 3 meetings, in the first, second and fourth month after delivery. In the first two meetings, we applied scales Edinburgh Post-Natal Depression Scale (EPDS), Self-Report Questionnaire (SRQ- 20) and Postpartum Bonding Questionnaire (PBQ). The cutoff was adopted for the EPDS ≥ 12. For data analysis, we performed Poisson multiple regression with robust adjustment of variance model by backward stepwise selection procedure. All women included in the study assessed by signing the Informed Consent Form (ICF). The study was approved by the Ethics Committee on Human Research of the Federal University of Viçosa. The prevalence of symptoms of postpartum depression (PPD) was 14.5% (n = 29) in the first month after birth and 9% (n = 18) in the second. The incidence was 2, 3% (n = 4). In the first month after birth, were associated with symptoms of PPD the age younger than 20 years (PR = 2.42, CI = 1.21 to 4.81), vaginal delivery (PR = 2.04, CI = 1 0.03 to 4, 04) traumatic delivery (PR = 2.31, CI = 1.21 to 4.78), negative reaction from fellow with the news of the pregnancy (PR = 2.51, CI = 1.23 to 5 , 12) and have not received emotional support after delivery (PR = 2.72, CI = 1.30 to 5.72). In the second month, the associated variables were mother's negative reaction to the news of the pregnancy (PR = 2.49, CI = 1.05 to 5.90), maternal perception of the child cries more than other babies of the same age (PR = 3.12, CI 1.32 to 7.35) and insufficient support from the partner during pregnancy (PR = 4.28, CI = 1.84 to 9.94). The prevalence of abandonment of Exclusive Breastfeeding (EBF) in the first, second and fourth month after delivery was 53.57% (n = 90), 47.62% (n = 80) and 69.64% (n = 117), respectively, and its incidence in the fourth month compared to the first was 48.7%. In the second month after birth, were associated with the abandonment of EBF variables PPD symptoms (PR = 1.61, CI = 1.19 to 2.19) and traumatic delivery (PR = 1.40, CI = 1 0.02 to 2, 91). In the fourth month, the variables associated with abandonment of EBF were low maternal education (PR = 2.15, CI = 1.36 to 3.38), no own property (PR = 1.23, CI = 1.02 to 1.48) , have returned to work (PR = 1.33, CI = 1.09 to 1.63), not having received guidance on breastfeeding postpartum (PR = 1.21, CI = 1.01 to 1.45), reaction negative mother with the news of the pregnancy (PR = 1.29, CI = 1.09 to 1.52) and no help from the fellow with the child (PR = 1.33, CI = 1.04 to 1.70). The results of this study showed that screening of DPP in the postpartum first weeks is extremely important to prevent the complications of depression on maternal health and promote longer duration of EBF, especially among adolescents and mothers who do not receive support from her partner. Women need a supportive environment that protects and promotes breastfeeding at home, in the workplace and in health services.

Trata-se de estudo transversal realizado com 168 puérperas provenientes do setor de imunização da Policlínica Municipal de Viçosa. Realizou-se 3 encontros, no primeiro, segundo e quarto mês após o parto. Nos dois primeiros encontros, aplicou-se as escalas Edinburgh Post-Natal Depression Escale (EPDS), Self-Report Questionaire (SRQ-20) e Postpartum Bonding Questionaire (PBQ). O ponto de corte adotado para a EPDS foi ≥ 12. Para análise dos resultados, foi realizada regressão múltipla de Poisson com ajuste robusto da variância pelo modelo stepwise backward selection procedure. Todas as puérperas avaliadas aceitaram participar do estudo mediante assinatura do Termo de Consentimento Livre e Esclarecido (TCLE). O trabalho foi aprovado pelo Comitê de Ética em Pesquisa com Seres Humanos da Universidade Federal de Viçosa. A prevalência de sintomas de depressão pós-parto (DPP) foi de 14,5% (n=29) e 9% (n=18) no primeiro e segundo mês após o parto, respectivamente, sendo que a incidência foi de 2,3% (n=4). No primeiro mês após o parto, associaram-se com sintomas de DPP a idade inferior a 20 anos (RP=2,42; IC=1,21-4,81), parto normal (RP=2,04; IC=1,03- 4,04) e traumático (RP=2,31; IC=1,21-4,78), reação negativa do companheiro com a notícia da gestação (RP=2,51; IC=1,23-5,12) e não ter recebido apoio emocional no puerpério (RP=2,72; IC=1,30-5,72). No segundo mês, as variáveis associadas foram reação negativa da mãe com a notícia da gestação (RP=2,49; IC=1,05-5,90), percepção materna de que a criança chora mais do que os outros bebês da mesma idade (RP=3,12; IC 1,32-7,35) e apoio insuficiente do companheiro durante a gestação (RP=4,28; IC=1,84-9,94). A prevalência de abandono do Aleitamento Materno Exclusivo (AME) no primeiro, segundo e quarto mês após o parto foi de 53,57% (n=90), 47,62% (n=80) e 69,64% (n=117), respectivamente, e sua incidência no quarto mês em relação ao primeiro foi de 48,7%. No segundo mês após o parto, associaram-se com o abandono do AME as variáveis sintomas de DPP (RP = 1,61; IC = 1,19-2,19) e parto traumático (RP = 1,40; IC = 1,02-2,91). No quarto mês, associaram-se s menor escolaridade materna (RP = 2,15; IC = 1,36-3,38), não possuir imóvel próprio (RP = 1,23; IC = 1,02-1,48), ter voltado a trabalhar (RP = 1,33; IC = 1,09-1,63), não ter recebido orientações sobre amamentação no puerpério (RP = 1,21; IC = 1,01-1,45), reação negativa da mãe com a notícia da gestação (RP = 1,29; IC = 1,09-1,52) e ausência de ajuda do companheiro com a criança (RP = 1,33; IC = 1,04-1,70). Os resultados deste estudo mostraram que o rastreamento da DPP nas primeiras semanas de puerpério é extremamente importante para prevenir os agravos da depressão sobre a saúde materna e promover maior duração do AME, especialmente entre as mães adolescentes e que não recebem apoio do companheiro.
