The genioplasty are surgical procedure that aim to treat genial deformities. They carried trougth for main esthetic objectives, but we should hipothetized that they can improve labial posture e labial relationship that accompanies some dentofacial deformities. In sight of this, the osteotomy of the inferior border of mandible, the technique used in this paper is one that presents great versatility, much used for correction of three-dimensional deformities of the menton, due the possible angular variation of the ostetomy. The accomplishment for intraoral boarding minimized risks of bigger resorptions for the lesser displacement of soft tissues and consenquently, maintenance of the sanguineous supplement, with more aesthetic results. The effect of landslide of the inferior edge provides harmony for the movements of the menton. Diferent methods had been describe to fix the genial segments since wire osteosynthesis, plates and screws, positional screws, Kirschner wires, plates and screws resorbable. In the present paper we test two methods of stabilization of the advanced genial segment: wire fixation and positional screws and analyze radiographycally the remodeling process postoperative. A retrospective study was done with two groups of twenty patients ( 10 stabilized by means of wire fixation and 10 by positional screws) that done genioplasty procedure combined with other orthognatic procedure by means of sequential study of preoperative, immediate postoperative and long-term cephalograms(>6 months). The method of analysis was done by means of a tracing on the cephalograms and vertical and horizontal position of the genial segment was analyzed beside the osseous postoperative remodeling pattern. These results was submitted to statistical analysis to achieve a reliable method. There was no diference in stability between the two groups tested and the remodelation was equal in both groups. We can conclude that genioplasty is hight predictitable and stable procedure independent of fixation methods besides the remodeling pattern didn’t differ with kind of fixation of genial segments.
