Infecções por Serratia spp. em ambientes de terapia intensiva: uma revisão integrativa

Metodologia: a pesquisa bibliográfica foi efetuada nas bases de dados PubMed, SciELO e BVS aplicando conjuntos de preditores pré-estabelecidos. Resultados: 18 artigos foram inicialmente selecionados, dos quais 12 foram analisados. Relatos de infecções associadas à espécie S. marcescens foram predominantes. Contudo, relatos sobre infecções simultâneas com outras espécies também foram registrados. Todos os estudos foram realizados em Unidades de Terapia Intensiva, sendo em sua maioria (75%) relatados em Unidades de Terapia Intensiva Neonatais (UTIN). Quanto ao padrão epidemiológico, 83,3% dos estudos relataram surtos. Os quadros clínicos mai frequentes associados a Serratia spp. foram episódios de sepse (41,6%), bacteremia (33,3%), infecção no trato urinário e pneumonia (25%) e conjuntivite (16,6%). Fatores relacionados à aquisição de infecções por S. marcescens foram prematuridade (50%), uso de ventilação mecânica (33,3%), síndrome do desconforto respiratório (25%), baixo peso ao nascer, uso de nutrição parenteral e de cateter venoso central (16,6%). Conclusão: infecções por S. marcescens prevaleceram em prematuros e recém-nascidos devido às características desta população, que no geral são extremamente frágeis e apresentarem sistema imune imaturo. Ademais, procedimentos de suporte a vida utilizados em UTIN são fatores que predispõe infecções por S. marcescens. __________________________________________________________________________ ABSTRACTAim: the study aimed to analyze scientific reports on hospital infections associated with Serratia spp. and therefore producing an integrative review. Methods: a bibliographic search was carried out in the scientific repositories PubMed, SciELO and BVS applying pre-established arrays of predictors. Results: 18 papers were initially selected from which 12 were analyzed. Reports on infections associated with the specie Serratia marcescens were predominant. However, reports on simultaneous infections with other species were also described. All retrieved studies were carried out in Intensive Care Units (ICU) but the majority of reports (75%) were in Neonatal Intensive Care Unit (NICU). Concerning the epidemiological profile, 83.3% of reports described outbreaks. Frequent clinical picture associated with Serratia spp. infections were sepsis (41.6%), bacteremia (33.3%), urinary tract infection and pneumonia (25%) and conjunctivitis (16.6%). Factors associated with the contraction of S. marcescens infections were prematurity (50%), the use of mechanical ventilation (33.3%), respiratory discomfort syndrome (25%) and low weight in birth, the use of parenteral nutrition and of venous central catheter (16.6%). Conclusion: S. marcescens infection in hospital environment were prevalent in premature and new-born babies due to peculiar characteristics of this population what in general are extremely weakly and show immature immune system.
