O presente relato de caso teve como objetivo abordar a reabilitação com carga e estética imediata de dentes anteriores superiores, por meio de cirurgia guiada para instalação de implantes osseointegráveis e próteses fixas provisórias. Após a avaliação da condição bucal e das informações fornecidas por exames de imagem (radiografias panorâmica, periapicais e tomografia computadorizada) e modelos montados em articulador, foi definido o plano de tratamento. A primeira fase consistiu na adequação bucal através de tratamento periodontal, exodontias e restaurações provisórias em cimento de ionômero de vidro e resina composta. Como o paciente apresentava dimensão vertical reduzida, foi necessária a confecção e instalação de prótese parcial removível provisória mandibular, o que restabeleceu o espaço desejado para reabilitação sobre implantes. O enceramento diagnóstico orientou a produção do guia cirúrgico e seleção dos implantes osseointegráveis, assim como próteses fixas provisórias em resina acrílica para a substituição dos elementos 21 ao 24. O procedimento cirúrgico compreendeu a instalação de três implantes osseointegráveis, imediatamente após as exodontias das raízes e elementos daquela região. A prótese parcial fixa provisória foi reembasada, instalada sobre os implantes recém- instalados, recebendo o ajuste oclusal. O resultado funcional e estético foi satisfatório, alcançando os objetivos esperados. The present case report had as aim to approach the rehabilitation with immediate loading and esthetics of upper anterior teeth, through guided surgery for the installation of osseointegrable implants and provisional fixed prostheses. After evaluation of the oral condition and information provided by imaging exams (panoramic radiography, periapical and computed tomography) and dental casts on articulator, the treatment plan was defined. The first phase consisted of oral adequacy through periodontal treatment, dental extraction and provisional restorations in glass ionomer cement and composite resin. As the patient had a reduced vertical dimension, it was necessary to make and install a temporary removable partial mandibular prosthesis, which restored the space required for rehabilitation on implants. Diagnostic waxing guided the manufacture of the surgical guide and selection of osseointegrable implants, as well as provisional fixed prosthesis in acrylic resin for the replacement of elements 21 to 24. The surgical procedure included the installation of three osseointegrable implants, immediately after dental and root extractions of dental elements of that region. The provisional fixed partial denture was relinquished, installed on the newly installed implants, receiving the occlusal adjustment. The functional and aesthetic result was satisfactory, granting a higher quality of life and improving the patient's self-esteem.
