Objetivo: Caracterizar a influência da ventilação mecânica (VM) com PEEP sobre a função renal em pacientes internados em UTI. Método: Estudo longitudinal, prospectivo, quantitativo, descritivo, desenvolvido na UTI do Hospital Regional de Ceilândia. A casuística foi composta de 27 pacientes sob suporte de VM. Foram incluídos os pacientes com idade acima de 18 anos, sob suporte de VM com PEEP, sem história de disfunção renal prévia. E excluídos aqueles com insuficiência renal crônica. Os pacientes selecionados foram alocados em três grupos de acordo com o valor da PEEP programada no ventilador mecânico. A partir desta distribuição, os pacientes foram acompanhados até o término da internação na UTI para avaliação da influência dessa pressão sobre a função renal. Os dados foram expressos em frequência absoluta (n) e frequência relativa (%), em mediana e percentil 25 e 75. Valores de p < 0,05 foram considerados significativos. Resultados: A maioria (55,6%) dos pacientes foram classificados no estágio de lesão renal de acordo com a classificação RIFLE e uma maioria (45,8%) desses estavam sob VM com uma PEEP ˃5cmH2O e <10cmH2O e evoluíram com lesão renal aguda. Verificou-se que o APACHE II, apresentou associação significativa com disfunção renal (p=0,046). Concluiu-se que a ventilação mecânica invasiva mostrou tendência em influenciar a função renal dos pacientes no cenário da UTI. ________________________________________________________________________ ABSTRACTIntroduction: The frequent use of PEEP in ICU led to a large number of studies on the association between changes in cardiopulmonary hemodynamics and the occurrence of renal dysfunction during PEEP. Objective: To characterize the influence of mechanical ventilation with positive end-expiratory pressure (PEEP) on renal function in patients hospitalized in the Intensive Care Unit (ICU). Method: This was a longitudinal, prospective, quantitative, descriptive study. Was developed in the Intensive Care Unit of the Hospital Regional of Ceilândia.The casuistry consisted of 27 patients admitted to the intensive care unit under mechanical ventilatory support. Were included in the study patients over the age of 18 years, under support of mechanical ventilation with positive end-expiratory pressure ( PEEP ) and with no preview history of renal dysfunction (according to RIFLE Rating - stage risk). And patients with history of chronic renal insufficiency was excluded (glomerular filtration rate < 60mL/min/1.73m2). Selected patients were divided into three groups according to the programmed ventilator. From this distribution, patients were followed until the end of the ICU hospitalization to assess the influence of this pressure on renal function. Data were expressed as absolute frequency (n) and relative frequency (%), 25 and 75 percentil and median. P values < 0.05 were considered significant. Results: Most (55.6%) patients were classified as stage renal lesion due to RIFLE classification and a majority (45.8%) of these were under mechanical ventilation with a PEEP ˃ 5cmH2O and <10cmH2O and developed acute kidney injury. It was found that the APACHE II was significantly associated with renal dysfunction (p = 0.046).
