Dentre os transplantes dentários, os de pré-molar para região de incisivo central superior são os que apresentam as maiores taxas de sucesso. Descrição do caso: Paciente do sexo feminino, 18 anos, sofreu trauma dentário nos incisivos centrais superiores aos 6 anos. Após tratamento endodôntico de ambos, concluiu-se pela extração eletiva do dente 11 em função de complicação endo-perio e pela impossibilidade da colocação de uma prótese nesse dente. A ortodontista da paciente solicitou a extração de dois pré-molares inferiores. Foi realizado, então, o transplante do elemento 44 para região do incisivo central superior. Discussão: O transplante autógeno é um método biológico que permite a manutenção do periodonto, crista óssea, papilas interdentárias e da vitalidade pulpar. Visando um melhor prognóstico desse procedimento, fez-se uso de um modelo prototipado do dente doador, além da realização do transplante pelo método de dois estágios e utilização de contenção não rígida. Conclusão: Realizado com técnica correta e minimamente traumática, o transplante dentário de pré-molar para região de incisivo central superior é uma opção viável e apresenta taxas elevadas de sucesso. Among dental transplants, the transplants of premolars to the upper central incisor region are those with the highest rates of success. Case description: A female patient, 18, suffered dental trauma in the upper central incisors when she was 6 years old. After the endodontic treatment, it was decided to realize an elective extraction of the 11 tooth due to a perio-endo lesion and the impossibility of placing a fixed dental prosthesis on that tooth. The patient’s orthodontist requested the extraction of two mandibular premolars. Then, it was performed the transplantation of the 44 tooth to the upper central incisor region. Discussion: The autologous transplantation is a biological method that allows the maintenance of the periodontum, bone crest, interdental papilla and pulp vitality. To a better prognosis of this procedure it was used a protyped model of the donor tooth, besides the surgery transplantation in a two-stage technique and use of non-rigid fixation. Conclusion: Performed with a correct and minimally traumatic technique, tooth transplantation of premolar to the central incisor region is a viable option and has high success rates.
