Os referenciais foram trabalhos científicos que descreveram o aparecimento da Síndrome de Wernicke Korsakoff em alguns casos pós-cirúrgicos devido ao déficit de vitamina B1 sérica (tiamina) e outros estudos que comprovaram a melhora da qualidade de vida após a cirurgia bariátrica. Realizou-se um estudo transversal com 40 sujeitos, sendo 20 obesos mórbidos não-operados (grupo controle) e 20 obesos mórbidos submetidos à cirurgia bariátrica. Foram utilizados os seguintes testes neuropsicológicos: RAVLT, Stroop e Minimental. Aplicou-se a escala WHOQoL-bref para avaliar qualidade de vida. Os níveis séricos de tiamina foram dosados. Na comparação do desempenho nos testes neuropsicológicos entre os 2 grupos, no sub-item LOT do teste RAVLT, o grupo controle apresentou melhores resultados; no sub-item Atenção e Cálculo do Minimental o grupo cirurgia obteve melhores resultados. Na amostra estudada a comparação entre as variáveis Tempo pós cirúrgico e desempenho nos testes o teste RAVLT-subitens: A3, A4, A5, ∑A1A5 e TA5-TA1 mostrou que quanto maior o tempo pós-cirúrgico melhor o desempenho dos sujeitos; comparando o resultado dos testes e o nível de tiamina constatou-se que quanto menor o nível de tiamina melhor o desempenho dos sujeitos no sub item REC-A do teste RAVLT. Os níveis séricos de vitamina B1 entre os 2 grupos não mostrou diferença. Houve melhora na qualidade de vida para os operados. Pelos resultados obtidos observou-se melhora do desempenho cognitivo no grupo pós-cirúrgico, o que poderia ser explicado pela melhora da função respiratória causada pela redução de peso e conseqüentemente melhora da função cerebral. Não se explicam alterações cognitivas co-dependentes dos níveis de tiamina. Há espaço para outros estudos a respeito, considerando que a literatura descreve dificuldades de absorção de nutrientes em geral, seja por baixa ingesta, seja por menor absorção desses em função das modificações (encurtamento) do trânsito intestinal. _______________________________________________________________________________________ ABSTRACTThe research aimed at evaluating the quality of life of individuals submitted to bypass gastric surgery as well as the possible cognitive deficit the procedure might cause. This work used as reference previous publications describing the occurrence of the Wernicke-Korsakoff Syndrome in patients submitted to the bariatric surgery due to low levels of seric B1 vitamin (thiamine), and other texts affirming the improvement in the quality of life of individuals who had surgery. The research tested 40 individuals, 20 of them morbid obseses who had not had surgery yet - the so called control group - and another 20 who had already had the bariatric procedure. The neurological tests used were the RAVLT, the STROOP and the Minimental. The WHOQoL-bref was applied in order to evaluate the quality of life of individuals. The seric levels of thiamine were measured. When comparing the results of the neuropsychological tests for both groups, the Control Group presented better results at the sub-item LOT of the RAVLT test. As for the sub-item Attention and Calculation of the MMSE test, the Operated Group presented better results. For the studied sample, when comparing the variables Time Elapsed After Surgery and results from the RAVLT test, sub-items A3, A4, A5, ∑A1A5 and TA5-TA1; the outcome was the longer the time after the surgery, better were the results. When comparing the RAVLT test result, sub-item REC-A, to the level of thiamine, the result was the lower the level of thiamine the better were the results. There was no difference in the seric levels of thiamine between the two groups analyzed. The quality of life improved for those operated. The results obtained showed an improvement in cognition for the operated group. This may be explained by the better conditions of both circulatory and respiratory systems due to the weight loss, allowing better functioning of the neurological system. Cognitive dysfunctions were not linked to the level of thiamine in the individuals tested. Further studying must be done, due to the fact that many have described difficulties in the absorption of nutrients.
