Aspectos clínico-epidemiológicos dos pacientes com meningites bacterianas em um hospital universitário de referência em Alagoas

Resumo: | ABSTRACT: Introduction: The World Health Organization estimates the occurrence of about 1.2 million cases and 135 thousand deaths from meningitis annually in the globe. In Brazil, bacterial meningitis (MB) manifests endemic behavior, being the most important, due to the magnitude of its prevalence, potential to cause outbreaks and lethality. Objective: Describe the clinical-epidemiological aspects of MB cases notified at a reference university hospital in Alagoas. Methodology: Observational, descriptive, cross-sectional and retrospective study based on clinical records of patients admitted to Hospital Escola Hélvio Auto by MB from 2010 to 2019. Results: From the investigation of 551 medical records, it was obtained that 2013 was the year of greatest prevalence, the epidemiological profile was composed of children, male, mixed-ethnic individuals, preschool students, residents of Eastern Alagoas and predominantly of cases during winter and fall. In turn, the clinical profile was characterized by fever, headache and vomiting, a history of previous contact in a home environment, a high rate of hospitalization and a satisfactory evolutionary outcome. Conclusion: The investigation allowed to trace the clinical and epidemiological profile of MB in the state of Alagoas, as well as identifying that the serogroup C of meningococcus, despite having a vaccination with wide access to the population, has shown a significant increase in the number of cases in recent years. RESUMO: Introdução: A Organização Mundial da Saúde, calcula a ocorrência de cerca de 1,2 milhões de casos e 135 mil mortes por meningites anualmente no globo. No Brasil, as meningites bacterianas (MB) manifestam comportamento endêmico, sendo as mais importantes, devido à magnitude de sua prevalência, potencial de acarretar surtos e letalidade. Objetivo: Descrever os aspectos clínico-epidemiológicos dos casos de MB notificados em um hospital universitário de referência em Alagoas. Metodologia: Estudo observacional, descritivo, transversal e retrospectivo a partir dos registros clínicos dos pacientes internados no Hospital Escola Hélvio Auto por MB de 2010 a 2019. Resultados: A partir da investigação de 551 prontuários médicos, observou-se que 2013 foi o ano de maior prevalência, o perfil epidemiológico foi composto por crianças, do sexo masculino, pardas, estudantes pré-escolares, residentes do Leste Alagoano e com predominância durante o inverno e o outono. Por sua vez, o perfil clínico caracterizou-se por febre, cefaleia e vômitos, histórico de contato prévio em ambiente extradomiciliar, elevada taxa de internação hospitalar e desfecho evolutivo satisfatório. Conclusão: A investigação realizada permitiu traçar o perfil clínico-epidemiológico das MB em Alagoas, bem como identificou que o sorogrupo C do meningococo, apesar de contar com uma vacinação de amplo acesso à população, apresentou significativo aumento no número de casos nos últimos anos.
