Objective: to analyze the challenges faced by nurses in the risk classification of an urgency and emergency service. Method: exploratory and descriptive research, with a qualitative approach. Data collection took place in March 2019, through structured and individual interviews with nurses working at the Emergency Care Unit in the city of Caruaru, Pernambuco, Brazil. Bardin's content analysis was used. Results: data analysis was organized into three thematic categories: Nursing assistance in triage; Risk rating industry challenges; and Challenges of the tool that defines the risk classification. Respondents spoke of their professional routines, pointing out issues such as lack of understanding of the population, outdated protocol and disagreement with other team members, as the main challenges. Conclusion: the performance of nursing in risk classification was shown to be a complex process, which does not depend strictly on the protocol, but also on professionals prepared for its use, recognizing all the factors that involve the patient, for an efficient classification.
