Avaliar qualidade de vida, suporte social e impacto na família dos pacientes no período pré e pós transplante de fígado. Foi realizado um estudo longitudinal com os pacientes em lista de espera para transplante hepático e transplantados no Ambulatório de Transplante Hepático da Universidade Estadual de Campinas (Unicamp). A pesquisa foi desenvolvida nas seguintes etapas: (1) Coleta de dados relativos às características demográficas e clínicas. (2) Avaliação da qualidade de vida utilizando o SF36 ¿ em português (3) Avaliação de suporte social utilizando-se o inventário de rede de suporte social (SSNI em português). (4) Coleta de dados para avaliação do impacto na família utilizando o questionário: inventário de sobrecarga de Zarit (ZBI). Os dados foram analisados por meio de estatística descritiva, comparativa e correlacional, utilizando-se o teste de Wilcoxon, qui-quadrado e o coeficiente de correlação de Spearman, no software SPSS - versão 12. Foram entrevistados 120 pacientes e familiares (impacto na família) e caracterizou-se pelo predomínio do sexo masculino (75,8%), casados (81.6%), com idade média foi 55,3 ± 9.7 anos, e metade dos pacientes possuíam o ensino fundamental. Dos entrevistados, 40% são aposentados e 30% estão em licença médica. Em 23.3% dos casos, o diagnóstico foi causa viral e em 20.8% foi causa alcoólica. O escore MELD puro, dos pacientes foi em média 17,7 ± 5,4. Em 54,1% apresentaram encefalopatia e em 42,5% houve presença de ascite. Pré-transplante: Na análise multivariada, verificou-se que os pacientes com maior escore de capacidade funcional foram os com ensino médio. O maior escore de aspectos físicos foram os pacientes com maior receptividade e os pacientes com menor escore de aspectos físicos foram os com ascite. Os pacientes com menor escore de dor foram os com ansiedade. O menor escore de vitalidade foram os pacientes que relataram agressividade e com encefalopatia. Nos pacientes com menor escore de aspectos emocionais foram os com encefalopatia e com maior MELD. O maior escore de saúde mental foram os pacientes com alegria e os pacientes sem agressividade, sem ansiedade e sem encefalopatia. Os pacientes com maior escore de impacto na família foram os com encefalopatia e os com agressividade. Pós-transplante: verificou-se diferença em todos as 8 dimensões SF36, com aumento do escore no períodos pós-transplante em comparação ao período pré-transplante. No pré-transplante observou-se melhor qualidade de vida quando o paciente possuía maior escolaridade, quando tinha receptividade e alegria, não apresentando ansiedade, medo e nem agressividade. Os pacientes com encefalopatia e/ou ascite tiveram menor qualidade de vida. O maior impacto na família foi quando o paciente era solteiro, possuía renda de até 2 salários mínimos, era portador de encefalopatia e agressividade. Comparando os períodos pré e pós-transplante observamos redução no suporte social e menor impacto na família o que demonstrou o benefício do transplante na vida do cuidador e do paciente. Em relação a qualidade de vida, todas as dimensões apresentaram melhora no período pós-transplante quando comparadas com o período pré-transplanteAbstract: Several diseases can lead to the need for liver transplantation, producing alterations in quality of life. Quality of life, social support and the impact on the family of transplantation candidate patients are important questions to be evaluated. To evaluate quality of life, social support and impact on the family of patients in the pre and post liver transplantation period. A longitudinal study was carried out with patients on the waiting list for hepatic transplantation and transplanted at the Hepatic Transplant Outpatient Clinic at the State University of Campinas (Unicamp). The research was developed in the following steps: (1) Collection of demographic and clinical data. (2) Evaluation of the quality of life using the SF36 - in Portuguese (3) Evaluation of social support using the inventory of social support network - SSNI in Portuguese. (4) Data collection for family impact assessment using the Zarit Overload Inventory (ZBI) questionnaire. The data were analyzed through descriptive, comparative and correlational statistics, using the Wilcoxon test, chi-square test and the Spearman correlation coefficient, in SPSS software - version 12. A total of 120 patients and family members (family impact) were interviewed. They were male (75.8%), married (81.6%), average age 55.3 ± 9.7 years old, and half of the patients had the elementary school. Out of the interviewees, 40% are retired and 30% are on medical leave. In 23.3% of the cases, the diagnosis was viral cause and in 20.8% it was an alcoholic cause. The pure MELD score of the patients was on average 17.7 ± 5.4. In 54.1% they presented encephalopathy and in 42.5% there was presence of ascites. Pre-transplantation: In the multivariate analysis, it was verified that the patients with the highest functional capacity score were those with secondary education. The highest score of physical aspects were the patients with greater receptivity and the patients with lower scores of physical aspects were those with ascites. Patients with lower pain scores were those with anxiety. The lowest vitality score were patients who reported aggression and encephalopathy. In patients with lower scores of emotional aspects were those with encephalopathy and with higher MELD. The highest mental health score were patients with joy and patients without aggression, without anxiety and without encephalopathy. The patients with the highest impact score in the family were those with encephalopathy and those with aggression. Post-transplantation: There was a difference in all 8 SF36 dimensions, with an increase in the score in the post-transplant periods compared to the pre-transplant period. In the pre-transplantation, a better quality of life was observed when the patient had a higher level of schooling, when he was receptive and cheerful, presenting no anxiety, fear or aggression. Patients with encephalopathy and / or ascites had lower quality of life. The greatest impact on the family was when the patient was single, had income of up to 2 minimum wages, had encephalopathy and aggression. Comparing the pre- and post-transplant periods, we observed a reduction in social support and a lower impact on the family, which demonstrated the benefit of transplantation in the life of the caregiver and the patient.
