Necrotizing sialometaplasia is an inflammatory process of benign character, found in the oral cavity mainly in the minor salivary glands located in the region of the hard palate, and may be unilateral or bilateral. Its etiology is variable, and can be of traumatic origin, alcoholism, smoking, among others, being the most common due to direct trauma. Clinically they present as deep, irregular, rapidly growing ulcerative lesions, with a long period for remission and complete healing, and may have malignant neoplasms as a diagnostic hypothesis. This paper aims to report a clinical case of necrotizing sialometaplasia caused by the use of ill-fitting total prosthesis. The patient was a 72-year-old female with an extensive ulcerated lesion measuring approximately 3 cm in diameter, irregular in shape and with well-defined borders on the left side of the hard palate. The treatment was performed with topical corticoid and photobiomodulation through low intensity laser, presenting satisfactory results within 30 days.
