Gastroduodenopancreatectomias: análise de 17 casos do serviço de cirurgia oncológica do hospital do câncer de Maringá

Resumo: | The gastroduodenopancreatectomy (GDP) is the recommended surgery for resection of peri-ampullary tumors and it is a procedure that requires a high technical level to be performed, given its high degree of complexity. Objectives: To analyze the profile of performed by the Oncological Surgery team at the Hospital do Câncer de Maringá in the last 5 years (2016 to 2020) and understand the experience of surgery by comparing it with other studies. Methods: Collection and retrospective analysis of data related to pre, peri, and postoperative medical records of patients 4 who underwent GDP. Results: The majority of patients were male, with a mean age of 61.3 years and race white and brown. The most prevalent comorbidity was systemic arterial hypertension, followed by diabetes mellitus. Smoking was the main life habit – found in 29% of cases. Jaundice and weight loss were the main signs and symptoms. The average time of surgery was 187.2 minutes. The tumors had an average size of 2.74 cm, located in most cases (58.8%) in the duodenal papilla. Margin involvement occurred in 11.8% of cases and lymph node involvement in 58.8%. The average number of lymph nodes removed was 18,9. Postoperative morbidity and mortality occurred in 70.6% and 17.6%, respectively. 52.9% of the patients underwent adjuvant chemotherapy and no one adjuvant radiotherapy. Conclusion: The surgical time and the average size of the tumors were below similar studies. The high mean number of lymph nodes removed was not correlated with greater detection of lymph node metastases. |

Objetivos: Analisar o perfil das GDPs realizadas pela equipe de Cirurgia Oncológica do Hospital do Câncer de Maringá nos últimos 5 anos (2016 a 2020) e compreender a experiência da cirurgia comparando-as com outros estudos. Métodos: Coleta e análise retrospectiva de dados referentes ao pré, peri e pós-operatório dos prontuários de pacientes que realizaram GDP. Resultados: A maioria dos pacientes era do sexo masculino, com média de idade de 61,3 anos e cor branca e parda. A comorbidade mais prevalente foi a hipertensão arterial sistêmica, seguida pela diabetes mellitus. O tabagismo foi relatado em 29% dos casos. Icterícia e perda de peso foram os principais sinais e sintomas. O tempo médio das cirurgias foi de 187,2 minutos. Os tumores tiveram tamanho médio 2,74cm, localizando na maioria dos casos (58,8%) na papila duodenal. O comprometimento das margens ocorreu em 11,8% dos casos e o acometimento linfonodal em 58,8%. A média de linfonodos ressecados foi de 18,9. A morbidade e a mortalidade pós-operatória ocorreram em, respectivamente, 70,6% e 17,6%. 52,9% dos pacientes realizaram quimioterapia adjuvante e não foram relatados pacientes submetidos à radioterapia adjuvante. Conclusão: O tempo cirúrgico e o tamanho médio dos tumores foram abaixo de estudos similares.
