Third molar extraction is a common procedure performed by dentists, its indications are the presence of cysts, carious lesions, periodontal disease, pericoronitis and others. During surgical planning, it is necessary to perform imaging exams to verify the relationship of the tooth with other anatomical structures. Thus, this study aimed to compare the use of panoramic radiography and cone beam computed tomography imaging in the preparation of lower third molar extractions. The survey of articles in the Google Academic database was used as a methodology. The keywords used for the research were: "panoramic radiography", "cone beam computed tomography", "mandibular third molars", "surgery", "surgical planning", "panoramic radiography", "cone beam computed tomography", “mandibular third molars”, “surgery” and “surgical planning”. Regarding the time frame, there was no year restriction. Articles in English and Portuguese on the proposed theme were included. Cone beam computed tomography was found to be essential in cases where panoramic radiography shows a close relationship between the teeth and adjacent structures such as the maxillary sinus and inferior alveolar nerve. Therefore, in these situations, conical beam computed tomography is recommended for better surgical planning.
