Vasos mesentéricos: descrição anatômica, desenvolvimento embrionário e principais patologias

Resumo: | This study aimed to describe the embryonic development and the distribution of human mesenteric vessels, relating them to some clinical and pathological conditions and their importance for the continuing medical education. A bibliographic survey was conducted mainly in the LILACS, PubMed and Scielo databases. The articles that didn’t meet the proposed theme, that didn’t have anatomical descriptions of humans or that weren’t in Portuguese, English or Spanish, were excluded. The final sample had 34 articles and 14 books, which were used for the literature review. The embryonic development of the gastrointestinal tract and the origin of the digestive tract and its blood vessels were described. The distribution of these blood vessels throughout the mesentery (upper mesenteric artery and vein, arterial arcades, justacolic arch [marginal artery], straight vessels, colic arteries and veins, inferior mesenteric artery and vein and sigmoid arteries and veins) is complex and reveals a system full of arterial and venous anastomosis. There were found nine diseases involving these vessels, presenting diverse pathophysiological mechanisms and clinical manifestations, most of the time, very similar, with nonspecific symptoms. This symptomatic condition requires high medical suspicion for diagnosing mesenteric ischemia. A low prevalence of studies addressing mesenteric vessel diseases was found, most possibly due to the diagnostic difficulty and the low incidence in medical practice. However, considering the fatal outcomes of many of these diseases, it is important to have these issues better developed in medical education, contributing to a more assertive and resolute approach. |

O levantamento bibliográfico foi realizado, majoritariamente, por meio das bases de dados LILACS, PubMed e Scielo. Foram excluídos os artigos que não atendiam à temática proposta, não continham descrições anatômicas em humanos ou não estavam nas línguas português, inglês ou espanhol. A amostra final contou com 34 artigos e 14 livros, utilizados para revisão bibliográfica. Descreveu-se o desenvolvimento embrionário do trato gastrintestinal e a origem do trato digestório e de seus vasos sanguíneos. A distribuição desses vasos pelo mesentério (artéria e veia mesentéricas superiores, arcadas arteriais, arco justacólico [artéria marginal], vasos retos, artérias e veias cólicas, artéria e veia mesentéricas inferiores, artérias e veias sigmóideas) é complexa e revela um sistema rico em anastomoses arteriais e venosas. Foram encontradas nove doenças que envolvem esses vasos, com mecanismos fisiopatológicos diversificados e manifestações clínicas frequentemente muito semelhantes, cursando com sintomas inespecíficos. Esse quadro sintomatológico exige elevada suspeita médica para chegar ao diagnóstico de isquemia mesentérica. A pesquisa constatou que há poucos estudos que abordam as doenças dos vasos mesentéricos, muito possivelmente devido à dificuldade diagnóstica e à baixa incidência na prática médica.
