Caracterização do desempenho em plataforma de força de atletas pós lesão de ligamento cruzado anterior: um estudo transversal

Resumo: | O objetivo do presente estudo foi caracterizar e comparar o desempenho de membros inferiores de atletas com lesão de ligamento cruzado anterior (LCA) como indicador de retorno à prática esportiva. A decisão de liberação pós lesão de ligamento cruzado anterior deve ser compartilhada entre a equipe de saúde responsável pelo acompanhamento da recuperação, treinadores e o atleta. Avaliações ao longo da recuperação dão suporte para que as decisões sejam tomadas com mais assertividade. Neste sentido, este estudo utilizou uma plataforma de força para medidas de desempenho de membros inferiores em atletas de alto rendimento por meio de teste de saltos e equilíbrio. Para este estudo observacional a amostra foi composta de 17 atletas de alto rendimento (vôlei, basquete, judô, ginástica artística e de trampolim) em processo de recuperação de lesão de LCA, sendo 8 do sexo feminino e 9 do sexo masculino, com média de idade de 20±0,5 anos. A avaliação do salto de contramovimento e equilíbrio foi realizada de forma bipodal e unipodal. Optou-se neste estudo pelo teste não-paramétrico de Mann-Whitney para identificar assimetrias nos resultados com duas pernas e somente uma e pelo teste de Wilcoxon para identificar assimetrias entre os membros lesionado e não-lesionado. Para identificar melhorias nas medidas de desempenho ao longo do processo de recuperação, utiliza-se o teste não-paramétrico de Kruskal-Wallis, além de analisar as correlações de Spearman com o tempo de forma numérica. No contramovimento a principal variável de desempenho (altura do salto) apresentou diferença significativa (p=0,03), bem como na taxa de desenvolvimento de força (RFD) que apresentou p=0,04. Para as medidas de equilíbrio nenhuma das comparações foi significativa. Logo, pode-se concluir que os resultados de equilíbrio independem de a perna utilizada ser a lesionada. O desempenho esperado é diferente quando comparado entre o membro lesionado e o não lesionado, sendo indicador para elaboração de estratégias de intervenções para diminuir assimetrias e recuperar o desempenho anterior à lesão, bem como garantir um retorno seguro aos treinos e jogos. No entanto os resultados obtidos devem ser analisados de forma individualizada, levando em consideração a evolução do tratamento. Não foi possível estabelecer características comuns no desempenho dos atletas em diferentes etapas de recuperação, apesar de assimetrias entre os membros lesionados e não lesionados estarem presentes. |

The decision to release after an anterior cruciate ligament injury must be shared between the health team responsible for monitoring recovery, coaches, and the athlete. Assessments throughout the recovery provide support for decisions to be made more assertively. In this sense, this study used a force plate to measure lower limb performance in high-performance athletes through jumping and balance testing. For this observational study, the sample of 17 high performance athletes (volleyball, basketball, judo, artistic gymnastics and trampoline) was in the process of recovering from ACL injuries, 8 of whom were female and 9 were male, with an average of age of 20±0.5 years. The countermovement and balance jump assessment were performed in a bipodal and unipodal manner. In this work, the Mann-Whitney non-parametric test was chosen to identify differences in results with two legs and only one and the Wilcoxon to identify differences between injured and uninjured limbs. To identify improvements in performance measures throughout the recovery process, the Kruskal- Wallis non-parametric test is used, in addition to analyzing Spearman's correlations with time numerically. In the countermovement, the main performance variable (heel height) showed a significant difference (p = 0.03), as well as in the force development rate (RFD), which presented p = 0.04. For balance measures, none of the comparisons was significant. Therefore, it can be concluded that the balance results are independent of the leg used being the injured one. The expected performance is different when compared between the injured and the uninjured limb, being an indicator for developing intervention strategies to reduce asymmetries and recover the performance prior to the injury, as well as guaranteeing a safe return to training and games. However, the results obtained must be analyzed individually, considering the evolution of the treatment. O objetivo do presente estudo foi caracterizar e comparar o desempenho de membros inferiores de atletas com lesão de ligamento cruzado anterior (LCA) como indicador de retorno à prática esportiva. A decisão de liberação pós lesão de ligamento cruzado anterior deve ser compartilhada entre a equipe de saúde responsável pelo acompanhamento da recuperação, treinadores e o atleta. Avaliações ao longo da recuperação dão suporte para que as decisões sejam tomadas com mais assertividade. Neste sentido, este estudo utilizou uma plataforma de força para medidas de desempenho de membros inferiores em atletas de alto rendimento por meio de teste de saltos e equilíbrio. Para este estudo observacional a amostra foi composta de 17 atletas de alto rendimento (vôlei, basquete, judô, ginástica artística e de trampolim) em processo de recuperação de lesão de LCA, sendo 8 do sexo feminino e 9 do sexo masculino, com média de idade de 20±0,5 anos. A avaliação do salto de contramovimento e equilíbrio foi realizada de forma bipodal e unipodal. Optou-se neste estudo pelo teste não-paramétrico de Mann-Whitney para identificar assimetrias nos resultados com duas pernas e somente uma e pelo teste de Wilcoxon para identificar assimetrias entre os membros lesionado e não-lesionado. Para identificar melhorias nas medidas de desempenho ao longo do processo de recuperação, utiliza-se o teste não-paramétrico de Kruskal-Wallis, além de analisar as correlações de Spearman com o tempo de forma numérica. No contramovimento a principal variável de desempenho (altura do salto) apresentou diferença significativa (p=0,03), bem como na taxa de desenvolvimento de força (RFD) que apresentou p=0,04. Para as medidas de equilíbrio nenhuma das comparações foi significativa. Logo, pode-se concluir que os resultados de equilíbrio independem de a perna utilizada ser a lesionada. O desempenho esperado é diferente quando comparado entre o membro lesionado e o não lesionado, sendo indicador para elaboração de estratégias de intervenções para diminuir assimetrias e recuperar o desempenho anterior à lesão, bem como garantir um retorno seguro aos treinos e jogos. No entanto os resultados obtidos devem ser analisados de forma individualizada, levando em consideração a evolução do tratamento. Não foi possível estabelecer características comuns no desempenho dos atletas em diferentes etapas de recuperação, apesar de assimetrias entre os membros lesionados e não lesionados estarem presentes. |
