Osteomyelitis associated with Actinomyces sp. in the mandibular bone: case report

Resumo: The Actinomycosis is a rare infection caused by microorganisms of the genus Actinomyces. Its clinical presentation may resemble or mask other infectious processes, which makes the diagnosis complex and compromises therapeutic success. The aim of this work is to report a case of osteomyelitis caused by a bacterial infection, discuss the clinical manifestations, forms of diagnosis and treatment. This is a descriptive, retrospective and qualitative study, carried out using the direct observation technique. Male patient, leucoderma, 65 years old, attended on maxillofacial surgery ambulatory with pain complaint in anterior region of the mandible and history of extraction 5 months ago on region with episodes of drainage of purulent secretion. Physical examination revealed absence of tooth 31 and intact tissues, and radiographic examination was requested, where was observed bone sequestration in the region of alveolar bone, with hypothesis of osteomyelitis. Surgical treatment was recommended  with excision of the radiopaque area, associated with antibiotic therapy. Histopathological evaluation confirmed the infectious process associated with colonies of bacteria Actinomyces sp.; the patient progressed well with complete remission of the infectious process. Thus, histopathological confirmation is an important tool for the rapid diagnosis and establishment of effective treatment, considering that actinomycosis makes a differential diagnosis with several diseases and has high local destructive power.

Su presentación clínica puede parecerse o enmascarar otros procesos infecciosos, lo que hace que su diagnóstico sea complejo y comprometa el éxito terapéutico. El objetivo de este trabajo es informar un caso de osteomielitis debido a una infección bacteriana, discutir las manifestaciones clínicas, formas de diagnóstico y tratamiento. Este es un estudio descriptivo, retrospectivo, cualitativo realizado utilizando la técnica de observación directa. Paciente masculino, leucoderma, de 65 años, asistió al servicio de cirugía maxilofacial con dolor en la región anterior de la mandíbula y antecedentes de extracción durante 5 meses en la región con episodios de drenaje de secreción purulenta. El examen físico reveló la ausencia de dientes 31 y tejidos intactos, y se solicitó un examen radiográfico, donde se observó secuestro óseo en la región del hueso alveolar, con una hipótesis diagnóstica de osteomielitis. El tratamiento recomendado fue un procedimiento quirúrgico con escisión del área radioopaca, asociado con el uso de un agente antimicrobiano. La evaluación histopatológica confirmó el proceso infeccioso asociado con colonias de bacterias Actinomyces sp.; el paciente progresó bien con la remisión completa del proceso infeccioso. Por lo tanto, la confirmación histopatológica es una herramienta importante para el diagnóstico rápido y el establecimiento de un tratamiento efectivo, teniendo en cuenta que la actinomicosis hace un diagnóstico diferencial con muchas enfermedades y tiene un alto poder destructivo local.A actinomicose é uma infecção rara, causada por microorganismos do gênero Actinomyces. Sua apresentação clínica pode se assemelhar ou mascarar outros processos infecciosos, o que torna complexo seu diagnóstico e compromete o êxito terapêutico. O objetivo deste trabalho é relatar um caso de osteomielite decorrente de uma infecção bacteriana, discutir as manifestações clínicas, formas de diagnóstico e tratamento. Trata-se de um estudo descritivo, retrospectivo, qualitativo feito por meio da técnica de observação direta. Paciente do sexo masculino, leucoderma, 65 anos de idade, compareceu ao serviço de cirurgia bucomaxilofacial com queixa álgica em região anterior de mandíbula e histórico de exodontia há 5 meses na região com episódios de drenagem de secreção purulenta. Ao exame físico observou-se ausência do dente 31 e tecidos íntegros, sendo solicitado exame radiográfico, onde observou-se sequestro ósseo em região de osso alveolar, com hipótese diagnóstica de osteomielite. O tratamento preconizado foi procedimento cirúrgico com exérese da área radiopaca, associado ao uso de um agente antimicrobiano. Avaliação histopatológica confirmou o processo infeccioso associado a colônias de bactérias Actinomyces sp.; o paciente evoluiu bem com completa remissão do processo infeccioso.
