Induction of lactation in transgender women: An integrative literature review

Resumo: | Our objective is to review the literature regarding the induction of lactation in transgender women, seeking publications, in any year, that discuss the relevant aspects of this topic. This is an integrative review and its systematization took place through the following steps: a) identification of the guiding question, b) search in the digital literature, c) evaluation of the findings, d) interpretative analysis of the results, e) discussion with summarization of knowledge. For the search, combinations of the following indexing terms were used: lactation, breastfeeding, trans women and transgender women (and their English counterparts). Data collection was carried out during the month of September 2022 in the PubMed, LILACS and SciELO databases. Initially, 27 publications were identified during the search. After applying the inclusion and exclusion criteria, a total of 09 works were obtained for inclusion in this study. It is possible to observe that there is a scarcity of works on the subject in the literature. Breastfeeding is a gender affirmation strategy for trans women and should be encouraged whenever possible. Milk production is possible through a protocol that consists of: stimulation and expression of the breast tissue, use of anti-androgens and galactagogues and increase with subsequent reduction of serum levels of estradiol and progesterone. It is concluded, therefore, that the induction of lactation in transgender women is feasible, economical, accessible and potentially safe. However, it is necessary to encourage research on this topic in order to answer the several remaining questions. |

Esta es una revisión integradora y su sistematización se dio mediante de los siguientes pasos: a) identificación de la pregunta orientadora, b) búsqueda en la literatura digital, c) evaluación de los hallazgos, d) análisis interpretativo de los resultados, e) discusión con resumen de los conocimientos. Para la búsqueda, se utilizaron combinaciones de los siguientes términos de indexación: lactancia, lactancia materna, mujeres trans y mujeres transgénero (y sus equivalentes en inglés). La recolección de datos se realizó durante el mes de septiembre de 2022 en las bases de datos PubMed, LILACS y SciELO. Inicialmente, se identificaron 27 publicaciones durante la busca. Después de aplicar los criterios de inclusión y exclusión, se obtuvieron un total de 09 trabajos para su inclusión en este estudio. La recolección de datos se realizó durante el mes de septiembre de 2022 en las bases de datos PubMed, LILACS y SciELO. Inicialmente, se identificaron 27 publicaciones durante la busca. Después de aplicar los criterios de inclusión y exclusión, se obtuvieron un total de 09 trabajos para su inclusión en este estudio. Es posible observar que hay escasez de trabajos sobre el tema en la literatura mundial. La lactancia materna es una estrategia de afirmación de género para las mujeres trans y debe fomentarse siempre que sea posible. La producción de leche es posible a través de un protocolo que consiste en: estimulación y expresión del tejido mamario, uso de antiandrógenos y galactogogos y aumento con posterior reducción de los niveles séricos de estradiol y progesterona. Se concluye, por tanto, que la inducción de la lactancia en mujeres transgénero es factible, económica, accesible y potencialmente segura. Sin embargo, es necesario impulsar la investigación sobre este tema para dar respuesta a los diversos interrogantes restantes que existen.Objetiva-se revisar a literatura no que diz respeito à indução da lactação em mulheres transgênero, buscando publicações, em qualquer ano, que tratem dos aspectos pertinentes a esse tema. Trata-se de uma revisão integrativa e sua sistematização deu-se através das seguintes etapas: a) identificação da questão norteadora, b) busca na literatura digital, c) avaliação dos achados, d) análise interpretativa dos resultados, e) discussão com sumarização do conhecimento. Para a busca, utilizou-se combinações dos seguintes termos de indexação: lactação, amamentação, mulheres trans e mulher transgênero (e seus correlatos em inglês). A coleta de dados foi realizada durante o mês de setembro de 2022 nas bases de dados PubMed, LILACS e SciELO. Inicialmente, 27 publicações foram identificadas durante a busca. Após a aplicação dos critérios de inclusão e de exclusão, obteve-se um total de 09 trabalhos para inclusão nesse estudo. É possível observar que há uma escassez de trabalhos sobre o tema na literatura mundial. O aleitamento configura-se como uma estratégia de afirmação de gênero para a mulher transgênero e deve ser encorajado sempre que possível. A produção de leite é possível através de um protocolo que consiste em: estimulação e expressão do tecido mamário, uso de antiandrogênios e de galactagogos e aumento com posterior redução dos níveis séricos de estrogênio e de progesterona, respectivamente. Conclui-se, portanto, que a indução da lactação em mulheres transgênero é factível, econômica, acessível e potencialmente segura.
