Risk for mortality in preoperative myocardial revascularization

Resumo: To evaluate the risk of mortality in the preoperative period of myocardial revascularization through the application of EuroSCOREII by the nurse. Prospective, exploratory, quantitative study, developed in a private reference hospital in cardiac surgery, in Teresina-PI. The baseline population consisted of all preoperative patients with myocardial revascularization, aged 18 years, from August to 2018 January 2019. It included patients over 18 years of age, in the immediate preoperative period of myocardial revascularization undergoing surgery elective. Data was collected using the EuroSCOREII Scale, applied individually in the preoperative period. Mortality was verified after 30 days of surgery, to compare the established risk. The analysis of the study was through the EuroSCOREII, Excel and SPSS for the risk of mortality and the calculated risk and the mortality rate verified in the postoperative period of the patients were compared. The project was submitted to the ethics committee of the University Center Uninovafapi and received the approved opinion under 2,741,913. The socio-demographic profile of patients in the preoperative period of myocardial revascularization, 76 in total, showed that 82% were male. The mean age was 64.98 years and the highest number was between 60 and 69 years (41%). Mortality risk was divided into low (13%), medium (25%), medium high (31%), high (28%) and very high (3%). Patients with observed mortality were medium-high risk and belonging to the male gender. Mortality observed at 30 days was 2.63%. The prediction of mortality by EuroSCOREII was simple and applicable in myocardial revascularization in the studied Brazilian population and the mortality observed was close to the percentage of multicenter studies. This study contributes to the perception that this cardiac risk assessment score can be used by Nurses to identify risk of mortality and, through the calculated risk, to define nursing behaviors during cardiac surgeries and to improve the patient’s prognosis.

El estudio tuvo como objetivo evaluar el riesgo de mortalidad en el preoperatorio de la cirugía de revascularización coronaria mediante la aplicación de EuroSCORE II por parte de las enfermeras. Investigación prospectiva, exploratoria y cuantitativa en un hospital privado de referencia para cirugía cardíaca, en Teresina, Piauí, Brasil. Todos los pacientes en revascularización miocárdica preoperatoria, mayores de 18 años, participaron en seis meses. Incluyendo la observación de la mortalidad postoperatoria, 30 días después de la cirugía. Se utilizó la escala de riesgo EuroSCORE II. El análisis realizado por EuroSCORE II, Excel y SPSS para el riesgo de mortalidad y comparó la tasa de mortalidad en el postoperatorio. Opinión del Centro Universitario Uninovafapi: 2741913. Los resultados mostraron que, de los 76 pacientes en el grupo estudiado, la mayoría eran hombres (82%; n = 62). La edad oscilaba entre 36 y 83 años, con un promedio de 64.98. años, con la mayoría del grupo clasificada como de riesgo medio alto (31%; n = 24). La mortalidad observada en el seguimiento de este grupo de estudio, hasta 30 días después de la operación, ocurrió en dos pacientes (2.63%) del total encuestado. La predicción de mortalidad por EuroSCORE II demostró su aplicación en la revascularización miocárdica en la región brasileña estudiada y la mortalidad observada fue cercana al porcentaje de EuroSCORE II en el estudio multicéntrico. Pesquisa prospectiva, exploratória, quantitativa, realizada em Hospital privado de referência em cirurgia cardíaca, em Teresina, Piauí, Brasil. Participaram todos os pacientes em pré-operatório de Revascularização do Miocárdio, maiores de 18 anos, no período de seis meses. Incluindo a observação da mortalidade no pós-operatório, após 30 dias da realização da cirurgia. Utilizou-se a Escala de risco EuroSCOREII. A análise pelo EuroSCOREII, Excel e SPSS para o risco de mortalidade e o comparou a taxa de mortalidade no pós-operatório. Parecer do Centro Universitário Uninovafapi: 2741913. Os resultados demonstraram que, dos 76 pacientes do grupo estudado, a maioria era do sexo masculino(82%; n=62), A idade variou entre 36 a 83 anos, com média de 64,98 anos, sendo a maioria do grupo ficando classificado em risco médio alto(31%; n=24). A mortalidade observada em acompanhamento desse grupo de estudo, até 30 dias de pós-operatório ocorreu em dois pacientes (2,63%)do total pesquisado. A predição de mortalidade pelo EuroSCOREII demonstrou aplicação em Revascularização do Miocárdio na região brasileira estudada e a mortalidade observada estava próxima do percentual EuroSCOREII no estudo multicêntrico.
