Nursing care for patients submitted to cine coronary angiography and the nurse’s role in the vascular sheath removal

Resumo: Objective: This study aims at reviewing the nursing role based on recent literature regarding patients submitted to cine coronary angiography, emphasizing arterial sheath removal procedure. Methods: This is an intuitive and narrative literature review, with a qualitative approach, using scientific articles published preferably in the last 5 years, available in full in Portuguese or English, in Scientific Electronic Library (Scielo), PubMED, and Biblioteca Virtual em Saúde (BVS) databases. Results: Nursing professionals provide complex and essential care for the maintenance of life in patients subjected to cine coronary angiography during every moment of their hospitalization. Brazilian nurses, when trained, have the autonomy and ability to proceed with arterial sheath removal after the percutaneous endovascular procedure in the catheterization laboratory. There are new vascular closure devices that can decrease hospitalization time in femoral punction cases. However, radial punction is still associated with a lesser adverse event rate. Evidence-based practices are key in preventing iatrogenics. Final considerations: There are several ways of removing the arterial sheath and reaching hemostasy. All of them require knowledge and experience in the best ways of providing nursing care and performing techniques, aiming for the patient to have the shortest possible recovery time, without increasing the risk to which he is subjected.

Métodos: Esta es una revisión narrativa e intuitiva de la literatura, enfoque cualitativo, utilizando artículos publicados preferentemente en los últimos 5 años, disponible en su totalidad en portugués o inglés, en bases de datos científicas como Biblioteca Electrónica en Línea (Scielo), PubMed y Biblioteca Virtual en Salud (BVS). Resultados: La enfermería ofrece cuidados complejos y esenciales para el mantenimiento de la vida de los pacientes sometidos a cinecoronariografía durante todos los momentos de su estancia hospitalaria. El enfermero calificado y capacitado tiene autonomía y capacidad para proceder con el retiro del introductor tras procedimiento percutáneo en la unidad de hemodinámica. Hay nuevos dispositivos de oclusión vascular que causan la disminución del tiempo de hospitalización em casos de punción arteria femoral, sin embargo, la abordaje radial se asocia con menores tasas de complicaciones. El uso de la práctica basada en la evidencia es fundamental para prevenir eventos iatrogénicos. Consideraciones finales: Hay varias formas de retirar el introductor vascular y adquirir hemostasia. Todos requieren conocimiento y experiencia en la mejor aplicación de la atención y técnicas de enfermería, para que el paciente tenga el menor tiempo de recuperación posible, sin aumentar el riesgo al que está sometido.Objetivo: Revisar com base na literatura recente a atuação da enfermagem em relação ao paciente submetido à cinecoronariografia, dando ênfase ao procedimento de retirada de introdutor vascular.  Métodos: Trata-se de uma revisão narrativa e intuitiva da literatura, de abordagem qualitativa, utilizando artigos publicados preferencialmente nos últimos 5 anos, disponíveis na íntegra na língua portuguesa ou inglesa, nos bancos de dados da Scientific Eletronic Library Online (Scielo), PubMed e Biblioteca Virtual em Saúde (BVS). Resultados: A enfermagem presta cuidados complexos e essenciais para a manutenção da vida dos pacientes submetidos à cinecoronariografia durante todos os momentos de sua permanência hospitalar. O enfermeiro capacitado e treinado tem autonomia e habilidade para proceder com a retirada do introdutor após procedimento percutâneo na unidade de hemodinâmica. Existem novos dispositivos de oclusão vascular que diminuem o tempo de hospitalização nos casos de punção femural, no entanto, a via radial ainda é associada a menores taxas de intercorrências. O uso da prática baseada em evidência é fundamental na prevenção de iatrogenias. Considerações finais: Existem diversas formas de realizar a retirada do introdutor vascular e adquirir hemostasia. Todas exigem conhecimento e experiência na melhor aplicação dos cuidados e de técnicas de enfermagem.
