Risk screening and prophylaxis of venous thromboembolism in inpatient units: an observational study

Resumo: Objectives: to identify the existence of venous thromboembolism (VTE) prevention measures in inpatient units of a public health system university hospital and to apply a clinical record for the screening of VTE risks by nurses. Method: transversal, descriptive and quantitative approach study, carried out in units of a quaternary university hospital of the Public Health System, located in the city of Rio de Janeiro. A total of 43 nurses participated and 90 medical records of hospitalized patients were used. Data collection was performed through two instruments: a form and a "VTE risk and conduct assessment form". The collected data were organized in Excel spreadsheets and for data analysis simple descriptive and analytical statistics were used.  Results: It was identified that most nurses do not record prophylactic actions and do not know protocols or algorithms about VTE. The most frequent risk factors in patients were: age over 40 years, prolonged immobility and use of central catheter or Swan-Ganz. Conclusion: the existence of underreporting in the medical charts made by nurses limited or prevented prophylactic actions by the multidisciplinary health team, as well as there were no protocols or algorithms for VTE prevention for use by nurses in the hospitalization units.

Método: estudio descriptivo transversal con enfoque cuantitativo, realizado en unidades de internación de un hospital universitario cuaternario del Sistema de Salud Pública, ubicado en la ciudad de Río de Janeiro. Participaron 43 enfermeras y se utilizaron 90 historias clínicas de pacientes hospitalizados. La recopilación de datos se realizó mediante dos instrumentos: un formulario y un "formulario de evaluación de riesgo y conducta para la prevención de TEV". Los datos recopilados se organizaron en hojas de cálculo Excel y para el análisis de datos, se utilizaron estadísticas descriptivas simples y analíticas. Resultados: se identificó que la mayoría de las enfermeras no registran acciones profilácticas y desconocen los protocolos o algoritmos sobre TEV. Los factores de riesgo más frecuentes en los pacientes fueron: edad mayor de 40 años, inmovilidad prolongada y uso de un catéter central o Swan-Ganz. Conclusión: la existencia de subregistro en los registros médicos realizados por enfermeras limitó o evitó las acciones profilácticas por parte del equipo de salud multidisciplinario, y no hubo protocolos o algoritmos para prevenir el TEV para el uso de enfermeras en unidades de hospitalización.Objetivos: identificar a existência de medidas de prevenção de tromboembolismo venoso (TEV) em unidades de internação de um hospital universitário do Sistema Público de saúde e aplicar uma ficha clínica para o rastreamento de riscos de TEV por enfermeiros. Método: estudo transversal, descritivo e de abordagem quantitativa, realizado em unidades de internação de um hospital universitário quaternário do Sistema Público de saúde, localizado no município do Rio de Janeiro. Participaram 43 enfermeiros e foram utilizados 90 prontuários de pacientes internados. A coleta de dados foi realizada através de dois instrumentos: um formulário e uma “ficha de avaliação de riscos e condutas para prevenção de TEV”. Os dados coletados foram organizados em planilhas do programa Excel e para análise dos dados utilizou-se estatística descritiva simples e analítica. Resultados: Identificou-se que maioria dos enfermeiros não registram ações profiláticas e desconhecem protocolos ou algoritmos sobre TEV.
