Analysis of the electrocardiogram in face of the change in the position of the electrodes: controlled clinical trial

Resumo: Objective: To analyze the electrocardiographic record in the face of intervention in the placement of electrodes on the chest compared to the standard method. Method: Controlled clinical trial, single blind, conducted with 41 patients, who composed the control and intervention groups, generating records that were lauded by two health professionals, proficient in cardiology. Results: Eighty-two electrocardiograms were analyzed, guided by the control and intervention protocols, which included the description of correct and incorrect techniques, respectively. Most of the variables studied did not present a statistically significant difference regarding the reports analyzed when comparing the protocols applied. The exception was the P wave, which was negative in V1 when it should have been positive, as evidenced in the comparison between 14 pairs of electrocardiograms (P = 0.003). Conclusion: The incorrect positioning of the electrodes affected the recording of lead V1. Considering the twelve-lead electrocardiogram usually performed in clinical practice, the possible confusion due to the underlying anatomical landmarks of other procedures, such as, for example, the focus of cardiac auscultation, is highlighted. We suggest the development of additional and technological criteria for the recognition and detection of lead location errors, as well as professional training being a sine qua non condition for carrying out the electrocardiography exam. Método: Ensayo clínico controlado, simple ciego, realizado con 41 pacientes, que compusieron los grupos de control e intervención, generando registros que fueron elogiados por dos profesionales de la salud, competentes en cardiología. Resultados: Se analizaron 82 electrocardiogramas, guiados por los protocolos de control e intervención, que incluyeron la descripción de las técnicas correctas e incorrectas, respectivamente. La mayoría de las variables estudiadas no presentaron diferencia estadísticamente significativa respecto a los informes analizados al comparar los protocolos aplicados. La excepción fue la onda P, que fue negativa en V1 cuando debería haber sido positiva, como se evidenció en la comparación entre 14 pares de electrocardiogramas (P = 0,003). Conclusión: El posicionamiento incorrecto de los electrodos afectó el registro de la derivación V1. Teniendo en cuenta el electrocardiograma de doce derivaciones que habitualmente se realiza en la práctica clínica, se destaca la posible confusión debido a los puntos de referencia anatómicos subyacentes de otros procedimientos, como, por ejemplo, el foco de la auscultación cardíaca. Sugerimos el desarrollo de criterios adicionales y tecnológicos para el reconocimiento y detección de errores de localización de electrodos, así como la formación profesional como condición sine qua non para la realización del examen electrocardiográfico. Método: Ensaio clínico controlado, simples cego, realizado com 41 pacientes, que compuseram o grupo controle e intervenção, gerando registros que foram laudados por dois profissionais de saúde, proficientes em cardiologia. Resultados: Foram analisados 82 eletrocardiogramas, tendo como direcionamento os protocolos controle e intervenção, nos quais constavam a descrição das técnicas correta e incorreta, respectivamente. A maioria das variáveis estudadas não apresentou diferença estatística significativa quanto aos laudos analisados diante da comparação entre os protocolos aplicados. A exceção foi referente à onda P, apresentando-se negativa em V1 quando deveria estar positiva, evidenciada na comparação entre 14 pares de eletrocardiogramas (P = 0,003). Conclusão: O posicionamento incorreto dos eletrodos afetou o registro da derivação V1. Considerando-se o eletrocardiograma de doze derivações usualmente realizado na prática clínica, salienta-se quanto ao possível confundimento devido aos marcos anatômicos base de outros procedimentos, como, por exemplo, os focos de ausculta cardíaca. Sugere-se o desenvolvimento de critérios adicionais e tecnológicos para o reconhecimento e detecção de erros de localização do eletrodo, bem como a capacitação profissional ser condição sine qua non para a realização do exame de eletrocardiografia.
