Comparison between Transcranial Direct Current Stimulation protocols in Chronic Migraine (NEUROMIG)

Resumo: | Chronic migraine (CM) has a wide range of symptoms, with debilitating consequences for those individuals refractory to the conventional treatments. In this context, Transcranial Direct Current Stimulation (tDCS) is a promising technique, although there is still no consensus on the most appropriate protocol, which may vary in relation to the number and interval between the application sessions. The present work consists of a protocol to assess the clinical and cognitive effects of ETCC on CM in the form of a double-blind, randomized and placebo-controlled clinical trial. The sample will consist of women aged between 20 and 45 years with a diagnosis of CM. A sociodemographic questionnaire and instruments for assessing cognitive functions, affective aspects, and clinical evaluation of migraine will be applied. Participants will be randomly distributed among 3 groups: Group 1: will receive a daily tDCS session for 15 minutes; Group 2: will receive two daily tDCS sessions of 15 minutes, with an interval of 20 minutes between them; Group 3: will receive simulated stimulation (sham) for 15 minutes. The anode electrode will be placed on the left dorsolateral prefrontal cortex and the reference electrode on the contralateral supraorbital region. In the Groups 1 and 2 the current will have an intensity of 2mA and will be applied for 10 consecutive days. We expect that tDCS will be effective in reducing symptoms in CM, with a decreased in pain and stress, and an improvement in the cognitive performance. This study will help to define a more appropriate neuromodulation protocol, contributing to the clinical decision making in CM. |

En este contexto, la estimulación transcraneal de corriente continua (ETCC) se muestra como una técnica prometedora, aunque todavía no hay consenso sobre el protocolo más adecuado, que puede variar con relación al número e intervalo entre las sesiones de aplicación. El presente trabajo consiste en un protocolo para evaluar los efectos clínicos y cognitivos de ETCC en la MC en forma de un ensayo clínico doble ciego, aleatorizado y controlado con placebo. La muestra estará compuesta por mujeres de entre 20 y 45 años con diagnóstico de MC. Se aplicará un cuestionario sociodemográfico e instrumentos para evaluar las funciones cognitivas, los aspectos afectivos y la evaluación clínica de la migraña. Los participantes se distribuirán aleatoriamente entre 3 grupos: Grupo 1: recibirá una sesión diaria de ETCC durante 15 minutos; Grupo 2: recibirá dos sesiones diarias de ETCC de 15 minutos, con un intervalo de 20 minutos entre ellas; Grupo 3: recibirá estimulación simulada (sham) durante 15 minutos. El electrodo anódico se colocará en la corteza prefrontal dorsolateral izquierda y el electrodo de referencia en la región supraorbital contralateral. En los grupos 1 y 2, la corriente tendrá una intensidad de 2 mA y será aplicará durante 10 días consecutivos. Se espera que la ETCC sea eficaz para reducir el cuadro sintomático en la MC, con disminución del dolor, estrés y un mejor rendimiento cognitivo. A amostra será composta por mulheres com idade entre 20 e 45 anos com diagnóstico de MC. Serão aplicados um questionário sociodemográfico e instrumentos de avaliação das funções cognitivas, aspectos afetivos, e de avaliação clínica da migrânea. As participantes serão distribuídas aleatoriamente em 3 grupos: Grupo 1: receberá uma sessão diária de ETCC de 15 minutos; Grupo 2: receberá duas sessões diárias de ETCC de 15 minutos, com um intervalo de 20 minutos entre elas; Grupo 3: receberá estimulação do tipo simulada (sham) por 15 minutos. O eletrodo anódico será alocado na região do córtex pré-frontal dorsolateral esquerdo e o eletrodo de referência na região supra-orbital contralateral. Nos grupos 1 e 2 a corrente terá intensidade de 2mA e será aplicada durante 10 dias consecutivos. Espera-se que a ETCC seja eficaz na redução do quadro sintomático na MC, com diminuição da dor, do estresse e melhora no desempenho cognitivo.
