Effects of Immunomodulatory Nutritional Therapy in cancer patients who have undergone gastrointestinal surgery: a scoping review

Resumo: | Introduction: Malnutrition represents a determining factor in the clinical outcome of surgical patients, since it can negatively influence the postoperative period, favoring the onset of complications such as delayed wound healing, infections, increased hospital stay, and consequently, mortality. Gastrointestinal tract surgeries in oncologic patients present high rates of complications and morbidity and mortality. Immunomodulatory nutritional therapy has been frequently used in these patients for its high potential to modulate immune activity, often contributing to the improvement of immune function and, consequently, favoring the clinical evolution. Method: This is a scoping review that will take place from September 2021 to January 2022. Studies in English and Portuguese were reviewed, and the databases selected for the search were Latin American and Caribbean Health Sciences Literature (LILACS), MEDLINE/PUBMED, plus gray literature. Results: Thirty-eight potentially eligible publications were identified, and the final sample consisted of 11 studies that evaluated the clinical outcome of cancer patients who received immunomodulatory nutritional therapy in the perioperative period of gastrointestinal surgery. Conclusion: The studies have shown that offering immunomodulatory formulas orally or enterally to cancer patients who have undergone gastrointestinal surgery has brought benefits, reducing complications such as infections and length of stay and providing better results in albumin level as well as in relation to immune function, raising the quality of life of patients. Some points such as mortality and T-lymphocyte increase could not be proven, suggesting that further studies should be conducted. |

Las cirugías del tracto gastrointestinal en pacientes oncológicos presentan altas tasas de complicaciones y morbilidad y mortalidad. La terapia nutricional inmunomoduladora se ha utilizado frecuentemente en estos pacientes por su alto potencial para modular la actividad inmunitaria, contribuyendo a menudo a la mejora de la función inmunitaria y, en consecuencia, favoreciendo la evolución clínica. Método: Se trata de una revisión de alcance realizada en el periodo comprendido entre septiembre de 2021 y enero de 2022. Se revisaron los estudios en inglés y portugués, y las bases de datos seleccionadas para la búsqueda fueron Latin American and Caribbean Health Sciences Literature (LILACS), MEDLINE/PUBMED, además de la literatura gris. Resultados: Se identificaron 38 publicaciones potencialmente elegibles, y la muestra final se compuso de 11 estudios que evaluaron el beneficio clínico de los pacientes oncológicos que recibieron terapia nutricional inmunomodulador en el período perioperatorio de la cirugía gastrointestinal. Conclusión: Los estudios han demostrado que ofrecer fórmulas inmunomoduladoras por vía oral o enteral a los pacientes con cáncer sometidos a cirugía gastrointestinal ha aportado beneficios, reduciendo complicaciones como las infecciones y la duración de la estancia y proporcionando mejores resultados en el nivel de albúmina, así como en relación con la función inmunitaria, aumentando la calidad de vida de los pacientes. Algunos puntos, como la mortalidad y el aumento de los linfocitos T, no pudieron ser probados, lo que sugiere que deberían realizarse más estudios.Introdução: A desnutrição representa um fator determinante no desfecho clinico dos pacientes cirúrgicos, uma vez que pode influenciar negativamente o pós-operatório, favorecendo o aparecimento de complicações como retardo na cicatrização das feridas, infecções, aumento no tempo de internação e consequentemente na mortalidade. As cirurgias do trato gastrintestinal em pacientes oncológicos apresentam altas taxas de complicações e morbimortalidade. A terapia nutricional imunomoduladora vem sendo frequentemente utilizada nesses pacientes pelo alto potencial de modular a atividade imune, podendo, muitas vezes, contribuir para a melhora da função imunológica e, consequentemente, favorecer a evolução clínica. Método: Trata-se de uma revisão de escopo realizada no período de setembro de 2021 a janeiro de 2022. Foram revisados estudos nos idiomas: inglês e português e, as bases de dados selecionadas para a busca foram a Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), MEDLINE/PUBMED, acrescidas de literatura cinzenta. Resultados: Foram identificadas 38 publicações potencialmente elegíveis, sendo a amostra final composta de 11 estudos que avaliaram o desfecho clínico de pacientes oncológicos que receberam terapia nutricional imunomoduladora no perioperatório de cirurgia gastrintestinal. Conclusão: Os estudos mostraram que a oferta de fórmulas imunomoduladoras por via oral ou enteral para pacientes oncológicos que se submeteram à cirurgia gastrintestinal trouxe benefícios, reduzindo complicações como infecções e tempo de internação e proporcionando melhores resultados no nível de albumina assim como em relação à função imunológica, elevando a qualidade de vida dos pacientes.
