Nursing care in the immediate postoperative period in cardiac transplant

Resumo: Introduction: Cardiac surgeries are described as complex interventions requiring adequate treatment in all surgical phases. General objective: to conduct a literature review to investigate the conduct of the nursing professional in the immediate postoperative period in heart transplantation. Methodology: for the development of this study, a literature review was conducted with a qualitative approach, opting only for publications with less than 5 years indexed in the journals in the health area. Result: the main complications that patients in the postoperative period by heart transplantation may face in their hospitalization time are, acute pain, impaired gas exchange, hypothermia, decreased cardiac output and risk for fluid volume desiquilibrium,cardiacrritia, hemodynamic instability, acute myocardial infarction. Pulmonary presentations may be atelectasis, pneumonia, episode of hypoxemia, and acute respiratory distress syndrome.  Therefore, nurses are extremely important because the professional acts in hemodynamic evaluation, evaluation of renal function and water balance; prevention of infections; detection and treatment of rejection; and ensuring the adequate supply of nutrients, in addition to providing human and material resources. Being responsible for caring, controlling and observing the patient, taking into account the complexity of the surgery.

Objetivo general: realizar una revisión bibliográfica para investigar la conducta del profesional de enfermería en el postoperatorio inmediato en el trasplante de corazón. Metodología: para el desarrollo de este estudio, se llevó a cabo una revisión bibliográfica con un enfoque cualitativo, optando sólo por publicaciones con menos de 5 años indexadas en las revistas del área de salud. Resultado: las principales complicaciones que los pacientes en el período postoperatorio por trasplante de corazón pueden enfrentar en su tiempo de hospitalización son, dolor agudo, cambio de gas deteriorado, hipotermia, disminución de la producción cardíaca y riesgo de desiquilibrio de líquido, cardiacrritia, inestabilidad hemodinámica, infarto agudo de miocardio. Las presentaciones pulmonares pueden ser atelectasia, neumonía, episodio de hipoxemia y síndrome de dificultad respiratoria aguda.  Por lo tanto, las enfermeras son extremadamente importantes porque los actos profesionales en la evaluación hemodinámica, evaluación de la función renal y el equilibrio hídrico; prevención de infecciones; detección y tratamiento del rechazo; y garantizar el suministro adecuado de nutrientes, además de proporcionar recursos humanos y materiales. Ser responsable de cuidar, controlar y observar al paciente, teniendo en cuenta la complejidad de la cirugía.Introdução: as cirurgias cardíacas são descritas como intervenções complexas exigem tratamento adequado em todas fases operatórias. Objetivo geral: realizar uma revisão da literatura para investigar a conduta do profissional de enfermagem no pós-operatório imediato no transplante cardíaco. Metodologia: para o desenvolvimento deste estudo foi realizada uma revisão da sistemática a com uma abordagem qualitativa, optando-se apenas por publicações com menos de 5 anos indexadas nas revistas na área de saúde. Resultado: as principais complicações mais comuns que os pacientes em pós-operatório por transplante cardíaco podem enfrentar em seu tempo de internação são, dor, dificuldade na troca de gases, débito cardíaco diminuído e risco para desequilíbrio de volume de líquido, arritmia cardíaca, instabilidade hemodinâmica e hipotermia. As apresentações pulmonares podem ser atelectasias, pneumonias, episódio de hipoxemia e a síndrome da angústia respiratória aguda. Portanto, o enfermeiro é extremamente importante, pois o profissional atua na avaliação hemodinâmica, avaliação da função renal e balanço hídrico; prevenção de infecções; detecção e tratamento de rejeição e garantir o suprimento adequado de nutrientes, além de fornecer recursos humanos e materiais.
