Sclerotherapy of intraoral hemangioma: case series

Resumo: Hemangiomas are benign lesions of common vascular origin in the head and neck region, but rarely arise in the oral cavity. The management and knowledge of the diagnostic techniques are fundamental for the differential diagnosis. Monoethanolamine oleate, as a sclerosing agent, promotes an inflammatory reaction leading to endothelial fibrosis, being effective in the treatment of benign vascular lesions of the oral cavity; besides the adverse reactions are of low intensity and little reported in the literature; showing an alternative to surgical treatment. The purpose of this study was to report a series of cases of patients undergoing sclerotherapy and to discuss the results of treatment of benign oral vascular lesions with monoethanolamine oleate. Four patients seen at the Bucomaxillofacial Surgery and Traumatology servisse of the Oswaldo Cruz University Hospital (CTBMF / HUOC / UPE) – Recife/PE with oral lesion complaint. After detailed clinical examination and the use of vitro pressure, hemangiomas diagnoses were obtained opting for treatment based on sclerotherapy with monoethanolamine oleate presenting satisfactory clinical resolution. In the cases reported here, the sclerosing agent has proved to be effective for the treatment of hemangiomas in the oral cavity.

El manejo y conocimiento de las técnicas diagnósticas son fundamentales para el diagnóstico diferencial. El oleato de monoetanolamina, como agente esclerosante, promueve una reacción inflamatoria que conduce a la fibrosis endotelial, siendo eficaz en el tratamiento de lesiones vasculares benignas de la cavidad oral; además, las reacciones adversas son de baja intensidad y poco reportadas en la literatura; mostrándose como una alternativa al tratamiento quirúrgico. El propósito de este estudio fue reportar una serie de casos de pacientes sometidos a escleroterapia y discutir los resultados del tratamiento de lesiones vasculares bucales benignas con oleato de monoetanolamina. Cuatro pacientes se presentaron en el servicio de Cirugía y Traumatología Buco-Maxilofacial del Hospital Universitário Oswaldo Cruz (CTBMF / HUOC / UPE) - Recife / PE con una queja de lesión bucal. Tras un examen clínico detallado y el uso de presión vitro, se obtuvo el diagnóstico de hemangiomas al optar por un tratamiento basado en escleroterapia con oleato de monoetanolamina que mostró resolución clínica satisfactoria. En los casos reportados aquí, el agente esclerosante fue efectivo para el tratamiento de hemangiomas en la cavidad oral.Hemangiomas são lesões benignas de origem vascular comuns na região da cabeça e pescoço, mas dificilmente surgem em cavidade bucal. O manejo e o conhecimento das técnicas diagnósticas são fundamentais para o diagnóstico diferencial. O oleato de monoetanolamina, como agente esclerosante, promove uma reação inflamatória levando a fibrose endotelial, sendo eficaz no tratamento de lesões vasculares benignas da cavidade bucal; além das reações adversas serem de baixa intensidade e pouco relatadas na literatura; mostrando-se uma alternativa ao tratamento cirúrgico. Este trabalho teve por proposito relatar uma série de casos de pacientes submetidos à escleroterapia e discutir os resultados do tratamento de lesões vasculares bucais benignas com oleato de monoetanolamina. Quatro pacientes compareceram no serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital Universitário Oswaldo Cruz (CTBMF/HUOC/UPE) – Recife/PE com queixa de lesão oral. Após exame clínico detalhado e a utilização da vitropressão foram obtidos os diagnósticos de hemangiomas optando-se pelo tratamento à base de escleroterapia com oleato de monoetanolamina apresentando satisfatória resolução clínica.
