The use of the pronation technique in mechanical ventilation

Resumo: | The use of prone mechanical ventilation has recently been incorporated into clinical practice, after studies have shown that the loss of lung volume in supine patients with acute respiratory distress syndrome is directly related to severity. This study aimed to analyze the use of the pronation technique in patients undergoing mechanical ventilation. The study is a bibliographic research of the integrative literature review type. The search for studies was performed using the PICO strategy, which represents an acronym for Patients (P), Intervention (I), Comparison (C) and Outcomes (O-outcomes). Data collection took place in August 2021 in the databases of the National Library of Medicine and the Virtual Health Library (VHL). 14 articles were included in this review. As for the results, it was shown that the main benefits of pronation are: improving oxygenation and minimizing harmful ventilation; avoid the use and complications of neuromuscular blockade; improve arterial blood oxygen pressure and saturation level in patients, among others. Despite the presence of complications, such as pressure injuries, which were relatively common, pronation is associated with a reduction in mortality and its use is highly recommended, since complications are relatively minor compared to the benefits of this practice. However, in the protocol of this technique it is recommended that protection measures be adhered to. Therefore, it is up to the team to assess the best approach to treat and monitor clients who are using mechanical ventilation. |

Este estudio tuvo como objetivo analizar el uso de la técnica de pronación en pacientes sometidos a ventilación mecánica. El estudio es una investigación bibliográfica del tipo revisión integradora de la literatura. La búsqueda de estudios se realizó utilizando la estrategia PICO, que representa un acrónimo de Pacientes (P), Intervención (I), Comparación (C) y Resultados (O-resultados). La recolección de datos tuvo lugar en agosto de 2021 en las bases de datos de la Biblioteca Nacional de Medicina y la Biblioteca Virtual en Salud (BVS). Se incluyeron 14 artículos en esta revisión. En cuanto a los resultados, se demostró que los principales beneficios de la pronación son: mejorar la oxigenación y minimizar la ventilación nociva; evitar el uso y las complicaciones del bloqueo neuromuscular; mejorar la presión arterial de oxígeno y el nivel de saturación en los pacientes, entre otros. A pesar de la presencia de complicaciones, como las lesiones por presión, que eran relativamente frecuentes, la pronación se asocia a una reducción de la mortalidad y su uso es muy recomendable, ya que las complicaciones son relativamente menores en comparación con los beneficios de esta práctica. Sin embargo, en el protocolo de esta técnica se recomienda seguir las medidas de protección. Por lo tanto, corresponde al equipo evaluar el mejor enfoque para tratar y monitorear a los clientes que utilizan ventilación mecánica.O uso da ventilação mecânica em pronação foi incorporado na prática clínica recentemente, após estudos demonstrarem que a perda de volume pulmonar nos doentes em decúbito dorsal com síndrome do desconforto respiratório agudo possui relação direta com a gravidade. Este estudo teve como objetivo analisar o uso da técnica de pronação em pacientes submetidos a ventilação mecânica. O estudo trata-se de uma pesquisa bibliográfica do tipo revisão integrativa da literatura. A busca por estudos foi realizada através da estratégia PICO, que representa um acrônimo para Pacientes (P), Intervenção (I), Comparação (C) e Desfechos (O-outcomes). A coleta de dados ocorreu no mês de agosto de 2021 nas bases de dados da National Library of Medicine e na Biblioteca Virtual em Saúde (BVS). Foram incluídos 14 artigos nesta revisão. Quantos aos resultados evidenciou-se que os principais benefícios da pronação, são: melhorar a oxigenação e minimizar a ventilação prejudicial; evitar o uso e complicações do bloqueio neuromuscular; melhorar a pressão de oxigênio no sangue arterial e o nível de saturação nos pacientes, entre outras. Apesar da presença de complicações, como as lesões por pressão que foram relativamente comuns, a pronação está associada com uma redução da mortalidade e seu uso é altamente recomendado, uma vez que, as complicações são relativamente ínfimas, quando comparadas aos benefícios desta prática. Entretanto, no protocolo desta técnica é recomendável adesão de medidas de proteção.
