Suspected sarcopenia, nutritional status and clinical outcome in hospitalized cardiac patients

Resumo: Objectives: To identify patients with suspected sarcopenia and to assess its association with nutritional status and clinical outcome. Patients and methods: Prospective study with cardiac patients admitted to the Coronary Unit. Subjective Global Assessment (SGA), nutritional assessment by body mass index (BMI), calf circumference (CC) measurement and screening for suspected sarcopenia by SARC-CalF were performed. Results: Forty patients (27H/13M;60.2±10 years) were studied, with an average length of stay (LS) of 13.4±7.6 days, and a 7.5% prevalence of deaths. Of the 37 patients screened by SARC-CalF, 09 (24.3%) were suspected of sarcopenia. By SGA, 29 (72.5%) patients were well nourished and 11 (27.5%) malnourished/nutritional risk. According to the BMI, 25 (62.5%) were overweight, 12 (30.0%) were eutrophic and 03(7.5%) lean. SGA was more sensitive to identify well-nourished (P=0.0003) and malnourished (P=0.0367) patients. Lower mean BMI (P=0.0049) and CC (P=0.0004) were observed in those suspected of sarcopenia. The group with suspected sarcopenia had more eutrophic patients (P=0.0106), and the group without suspected higher prevalence of overweight (P=0.0006). The death outcome was more present in those at nutritional risk/malnourished by SGA (P=0.0036). There was no difference between the groups regarding LS, presence of infection, readmission and death (P>0.05). Conclusion: There was an association of suspected sarcopenia with lower means of BMI and CC. Nutritional risk or malnutrition by SGA was associated with the occurrence of deaths, but there was no association of suspected sarcopenia with the clinical outcome.

Metodología: Estudio prospectivo con pacientes cardíacos ingresados en la Unidad Coronaria. Se realizó una Evaluación Global Subjetiva (SGA), una evaluación nutricional por índice de masa corporal (IMC), una medición de la circunferencia de la pantorrilla (LC) y un cribado de sospecha de sarcopenia mediante SARC-CalF. Resultados: Se estudiaron 40 pacientes (27H/13M;60,2±10 años), con tiempo de estancia (TI) 13,4±7,6 días y una prevalencia de defunciones del 7,5%. De los 37 pacientes examinados por SARC-CalF, 09 (24,3%) eran sospechosos de tener sarcopenia. Según la ASG, 29 (72,5%) pacientes estaban bien alimentados y 11 (27,5%) estaban desnutridos / riesgo nutricional. Por IMC, 25 (62,5%) tenían sobrepeso (EP), 12 (30,0%) eran eutróficos y 03 (7,5%) eran delgados. ASG fue más sensible para identificar pacientes bien nutridos (P=0,0003) y desnutridos (P=0,0367). Se observaron medias más bajas de IMC (P=0,0049) y CP (P=0,0004) en los sospechosos de sarcopenia. El grupo con sospecha de sarcopenia tenía más pacientes eutróficos (P=0,0106) y el grupo sin sospecha tenía una mayor prevalencia de EP (P=0,0006). El resultado muerte estuvo más presente en aquellos con riesgo nutricional/desnutrición según el ASG (P=0,0036). No hubo diferencias entre los grupos con respecto a la IT, la presencia de infección, el reingreso y la muerte (P>0,05). Conclusión: Hubo una asociación de sospecha de sarcopenia con menores medias de IMC y CC. Dos 37 pacientes triados pelo SARC-CalF, 09 (24,3%) tinham suspeita de sarcopenia. Pela ASG, 29 (72,5%) pacientes eram bem nutridos e 11 (27,5%) desnutridos/risco nutricional. Pelo IMC, 25 (62,5%) tinham excesso de peso (EP), 12 (30,0%) eram eutróficos e 03 (7,5%) magros. ASG foi mais sensível para identificar pacientes bem nutridos (P=0,0003) e desnutridos (P=0,0367). Observou-se menores médias de IMC (P=0,0049) e CP (P=0,0004) naqueles suspeitos de sarcopenia. O grupo com suspeita de sarcopenia apresentou mais pacientes eutróficos (P=0,0106), e o grupo sem suspeita maior prevalência de EP (P=0,0006). O desfecho óbito foi mais presente naqueles com risco nutricional/desnutrição pela ASG (P=0,0036). Não houve diferença entre os grupos em relação ao TI, presença de infecção, reinternação e óbito (P>0,05). Conclusão: Houve associação de suspeita de sarcopenia com menores médias de IMC e CP.
