Cross-sectional descriptive study of multidrug-resistant bacteria in the ICU environment of two medium-sized hospitals in the interior of São Paulo

Resumo: | The hospitals of Dracena (SCD) and Presidente Venceslau (SCPV) are the largest hospitals in the west region of the state of São Paulo and, due to the high number of patients seen in these hospitals, this work aimed to research multidrug-resistant bacteria in the ICU bed environment of these two hospitals. This is a descriptive and transversal observational research, where samples were collected from the environment of the 10 ICU beds of the SCD and the 10 ICU beds of the SCPV, in regions with frequent contact with hands and fomites. Isolated grown colonies were identified at the genus and species level. Antimicrobial susceptibility testing was performed following the recommendations of the Clinical and Laboratory Standards Institute (2021). Forty-five bacteria were isolated, 25 from SCD and 20 from SCPV. Most bacteria were Gram-negative bacilli (N = 27; 60%), followed by Staphylococcus aureus (N = 12; 26.7%) and Enterococcus faecalis (N = 6; 13.3%). Bacteria were found in the environment of the 10 ICU beds of the SCD and in nine of the 10 ICU beds of the SCPV. Most of the isolated bacteria showed resistance to several antimicrobials, being identified 34 MR, being 23 of SCD and 11 of SCPV. The isolates were grouped into seven groups of resistance phenotype, showing a possible clonal dispersion. The presence of MR in the hospital environment generates permanent concern, since they can infect hospitalized patients, especially after a breach of the protocol to prevent the transmission of infectious agents. This work will contribute to the improvement of hygiene protocols, offering more security to users of these hospitals. |

Esta es una investigación observacional descriptiva y transversal, donde muestras fueron recolectadas del ambiente de las 10 camas de UCI del SCD y de las 10 camas de UCI del SCPV, en regiones con frecuente contacto con manos y fómites. Las colonias cultivadas aisladas se identificaron a nivel de género y especie. Las pruebas de susceptibilidad antimicrobiana se realizaron siguiendo las recomendaciones del Clinical and Laboratory Standards Institute (2021). Se aislaron cuarenta y cinco bacterias, 25 de SCD y 20 de SCPV. La mayoría de las bacterias fueron bacilos gramnegativos (N = 27; 60 %), seguidos de Staphylococcus aureus (N = 12; 26,7 %) y Enterococcus faecalis (N = 6; 13,3 %). Se encontraron bacterias en el ambiente de las 10 camas de UCI del SCD y en nueve de las 10 camas de UCI del SCPV. La mayoría de las bacterias aisladas mostraron resistencia a varios antimicrobianos, identificándose 34 MR, siendo 23 de SCD y 11 de SCPV. Los aislados se agruparon en siete grupos de fenotipo de resistencia, mostrando una posible dispersión clonal. La presencia de MR en el ambiente hospitalario genera una preocupación permanente, ya que pueden infectar a los pacientes hospitalizados, especialmente luego de un incumplimiento del protocolo para prevenir la transmisión de agentes infecciosos. Esta obra contribuirá a la mejora de los protocolos de higiene, ofreciendo más seguridad a los usuarios de estos hospitales.As Santas Casas de Dracena (SCD) e Presidente Venceslau (SCPV) são os maiores hospitais da região Oeste Paulista e, devido ao elevado número de pacientes atendidos nestes hospitais, este trabalho teve por objetivo a pesquisa de bactérias multirresistntes nos ambiente de leito de UTI desses dois hospitais. Trata-se de uma pesquisa descritiva e transversal de caráter observacional, onde foram coletadas amostras do ambiente dos 10 leitos de UTI da SCD e dos 10 leitos de UTI da SCPV, em regiões de frequente contato com mãos e fômites. As colônias crescidas isoladas foram identificadas em nível de gênero e espécie. O teste de sensibilidade aos antimicrobianos foi realizado seguindo as recomendações do Clinical and Laboratory Standards Institute (2021). Foram isoladas 45 bactérias, sendo 25 da SCD e 20 da SCPV. A maioria das bactérias foi bacilos Gram-negativos (N = 27; 60%), seguida por Staphylococcus aureus (N = 12; 26,7%) e Enterococcus faecalis (N = 6; 13,3%). Foram encontradas bactérias no ambiente dos 10 leitos da UTI da SCD e em nove dos 10 leitos de UTI da SCPV. A maioria das bactérias isoladas apresentou resistência a vários antimicrobianos, sendo identificados 34 MR, sendo 23 da SCD e 11 da SCPV. Os isolados foram reunidos em sete grupos de fenótipo de resistência, mostrando uma possível dispersão clonal. A presença de MR no ambiente hospitalar gera preocupação permanente, uma vez que elas podem infectar pacientes internados, sobretudo após uma quebra do protocolo de prevenção de transmissão de agentes infecciosos.
