Experience of Balint groups in the medical course: Assessment of the importance and quality of the doctor-patient relationship experience

Resumo: | Introduction: The physician's ways of relating to the patient tend to adopt biomedical models, which are simpler and impersonal, to the detriment of biopsychosocial models, which are more complex and with greater emotional demand. Among the various interventions proposed for the development and facilitation of the doctor-patient relationship are the Balint groups. Objective: To evaluate the participation in Balint groups in the development of a patient-centered attitude in the context of the doctor-patient relationship in relation to non-participants. Methods: Qualitative study of a phenomenological nature, with a sample composed of 30 participants, including medical students and physicians. Data collection took place in 2021 through narrative interviews. Results: Six ways were found to characterize what is essential in the encounter with the patient: Empathy; Effective Communication; Biopsychosocial View; Time; Physician Availability; Infrastructure. Seven “units of meaning” characterized the difficulties encountered in the encounter with the patient: Limitations of Access to Health; Ineffective Communication; Insecurities of the assistant physician; Difficult Patients; Patient Expectation; Excessive Work Load; Addressing Sensitive Matters. Three “units of meaning” were used by both groups to exemplify when the meeting with the patient was successful: Understanding the health-disease process; Medical Contribution; Patient gratitude. Conclusion: It was noticed, from the interviews, similarities between the groups, which present an optimistic view regarding the human and integral care of their patients, even with different approaches. |

Entre las diversas intervenciones propuestas para el desarrollo y facilitación de la relación médico-paciente se encuentran los grupos Balint. Objetivo: Evaluar la participación en los grupos Balint en el desarrollo de una actitud centrada en el paciente en el contexto de la relación médico-paciente en relación a los no participantes. Métodos: Estudio cualitativo de carácter fenomenológico, con una muestra compuesta por 30 participantes, entre estudiantes de medicina y médicos. La recolección de datos ocurrió en 2021 a través de entrevistas narrativas. Resultados: Se encontraron seis formas de caracterizar lo esencial en el encuentro con el paciente: Empatía; Comunicación efectiva; Visión Biopsicosocial; Hora; Disponibilidad de médicos; Infraestructura. Siete “unidades de sentido” caracterizaron las dificultades encontradas en el encuentro con el paciente: Limitaciones de Acceso a la Salud; comunicación ineficaz; Inseguridades del médico asistente; Pacientes Difíciles; Expectativa del Paciente; Carga de trabajo excesiva; Abordar asuntos sensibles. Tres “unidades de significado” fueron utilizadas por ambos grupos para ejemplificar cuando el encuentro con el paciente fue exitoso: Comprensión del proceso salud-enfermedad; Contribución Médica; Agradecimiento paciente. Conclusión: Se percibió, a partir de las entrevistas, similitudes entre los grupos, que presentan una visión optimista en cuanto al cuidado humano e integral de su paciente, incluso con enfoques diferentes.Introdução: As formas do médico se relacionar com o doente tendem a adotar modelos biomédicos, que são mais simples e impessoais, em detrimento a modelos biopsicossociais, mais complexos e com maior demanda emocional. Objetivo: Avaliar a participação nos grupos Balint no desenvolvimento de atitude centrada na pessoa doente no contexto da relação médico-paciente em relação aos não participantes. Métodos: Estudo qualitativo de cunho fenomenológico, com amostra composta por 30 participantes incluindo acadêmicos de medicina e médicos. A coleta dos dados ocorreu em 2021 através de entrevistas narrativas. Resultados: Foram encontradas seis maneiras de caracterizar o que é essencial no encontro com o paciente: Empatia; Comunicação Efetiva; Visão Biopsicossocial; Tempo; Disponibilidade do Médico; Infraestrutura. Sete “unidades de sentido” caracterizaram as dificuldades encontradas no encontro com o paciente: Limitações de Acesso à Saúde; Comunicação Ineficaz; Inseguranças do médico-assistente; Pacientes Difíceis; Expectativa do Paciente; Carga Excessiva de Trabalho; Abordar Assuntos Delicados. Três “unidades de sentido” foram utilizadas por ambos os grupos para exemplificar quando o encontro com o paciente foi bem-sucedido: Entendimento do Processo saúde-doença; Contribuição Médica; Gratidão do Paciente.
