Mucormicosis: Estudio de una serie de casos clínico-patológicos

Resumo: | The aim of this study was to provide an overview of the possible presentations or forms of this rare, invasive fungal infection of the face, comparing it from diagnosis to treatment. This is an observational, descriptive, case series study, in which all cases diagnosed with fungal infections on the face were reviewed from the files of the Hospital Estadual de Urgências Governador Otávio Lage de Siqueira, Goiânia, GO, Brazil. Clinical data were obtained from patient records and radiographic characteristics were evaluated in each case. All cases were reviewed microscopically to confirm the diagnosis. Three cases of patients with mucormycosis fungal infection were identified, 1 in a woman and 2 in men. Both with common clinical manifestations: amaurosis, maxillary sinusitis, facial nerve involvement and extensive necrosis of the involved region. The diagnostic elucidation of the cases was carried out through microbiological examinations and computed tomography. They were conducted using a multimodal approach, including control of underlying predisposing factors, early administration of active antifungal agents at optimal doses, and complete removal of any infected tissue. In conclusion, mucormycosis infections are rare, invasive and usually fatal, with a low survival rate. It is commonly disseminated in patients with compromised immunity, particularly those with decompensated diabetes. There is still a lack of literature on the definitive management of this disease involving the stomatognathic system. This article emphasizes the need for correction of the underlying predisposing factor and early diagnosis with an aggressive multimodal treatment approach to provide the best chance of survival. |

Trata-se de um estudo observacional, descritivo, do tipo série de casos, no qual foram revisados todos os casos com diagnóstico de infecções fúngicas em face provenientes dos arquivos do Hospital Estadual de Urgências Governador Otávio Lage de Siqueira, Goiânia, GO, Brasil. Os dados clínicos foram obtidos dos prontuários dos pacientes e as características radiográficas foram avaliadas em cada caso. Todos os casos foram revisados microscópicamente para confirmar o diagnóstico. Foram identificados 3 casos de pacientes com infecção fúngica por mucormicose, 1 em mulher e 2 em homens. Ambos com manifestações clínicas comuns: amaurose, sinusite maxilar, comprometimento do nervo facial e necrose extensa de região envolvida. A elucidação diagnóstica dos casos foi realizada por meio de exames microbiológicos e tomografia computadorizada. Foram conduzidos por abordagem multimodal, incluindo o controle de fatores predisponentes subjacentes, administração precoce de agentes antifúngicos ativos em doses ideais e remoção completa de qualquer tecido infectado. Concluindo, as infecções por mucormicose são raras, invasivas e geralmente fatais, com baixa taxa de sobrevivência. É comumente disseminada em pacientes com imunidade comprometida, particularmente aqueles com diabetes descompensada. Ainda há escassez de literatura sobre o manejo definitivo desta doença envolvendo o sistema estomatognático. Este artigo enfatiza a necessidade da correção do fator predisponente subjacente e diagnóstico precoce com abordagem agressiva de tratamento multimodal para oferecer a melhor chance de sobrevivência.El objetivo de este estudio fue proporcionar una visión general de las posibles presentaciones o formas de esta rara infección fúngica invasiva de la cara, comparándola desde el diagnóstico hasta el tratamiento. Se trata de un estudio observacional, descriptivo, de serie de casos, en el que todos los casos diagnosticados con infecciones fúngicas en la cara fueron revisados del expediente del Hospital Estadual de Urgências Governador Otávio Lage de Siqueira, Goiânia, GO, Brasil. Los datos clínicos se obtuvieron de los registros de los pacientes y se evaluaron las características radiológicas de cada caso. Todos los casos fueron revisados microscópicamente para confirmar el diagnóstico. Se identificaron tres casos de pacientes con infección por hongos mucormicosis, 1 en una mujer y 2 en hombres. Ambos con manifestaciones clínicas comunes: amaurosis, sinusitis maxilar, afectación del nervio facial y necrosis extensa de la región involucrada. El esclarecimiento diagnóstico de los casos se realizó mediante exámenes microbiológicos y tomografía computarizada. Se llevaron a cabo utilizando un enfoque multimodal, que incluyó el control de los factores predisponentes subyacentes, la administración temprana de agentes antimicóticos activos en dosis óptimas y la eliminación completa de cualquier tejido infectado. En conclusión, las infecciones por mucormicosis son raras, invasivas y habitualmente mortales, con una baja tasa de supervivencia. Comúnmente se disemina en pacientes con inmunidad comprometida, particularmente aquellos con diabetes descompensada. Aún falta literatura sobre el manejo definitivo de esta enfermedad que involucra el sistema estomatognático.
