Functional evaluation after oclusal plate therapies and physiotherapy in patients with TMD: randomized clinical trial

Resumo: Objective: To evaluate the amplitude and pattern of mouth opening in patients diagnosed with Temporomandibular Disorder (TMD) who underwent treatment with Occlusal Plaque (PO) and Physiotherapy (F) after 1 and 3 months of treatment. Methodology: The mouth opening pattern and the mouth opening amplitude of 48 patients submitted to a randomized clinical trial and divided into 2 treatment groups were evaluated: occlusal plaque (PO; N = 23) and physical therapy (F; N = 25) . All patients were diagnosed with TMD according to axis 1 of the RDC / TMD (Research Diagnostic Criteria for Temporomandibular Disorders). The collected data were evaluated using the SPSS program using the Wilcoxon, Kruskal Wallis and Fisher's exact Chi Square tests. Results: PO and F showed similar performance in the opening pattern, maintaining the same pattern as the patient over the treatment time (P = 0.003). At the maximum opening amplitude without assistance, both therapies showed a significant improvement in amplitude gain in the initial time and after one month of therapy (P = 0.002), maintaining the amplitude until the third month, without presenting significant gain. Conclusion: PO and F showed similar performance for the treatment of TMD when analyzing the opening pattern and the mean maximum opening amplitude without assistance.

Metodología: Se evaluó el patrón de apertura bucal y la amplitud de apertura bucal de 48 pacientes sometidos a un ensayo clínico aleatorizado y divididos en 2 grupos de tratamiento: placa oclusal (PO; N = 23) y fisioterapia (F; N = 25) . Todos los pacientes fueron diagnosticados de TMD según el eje 1 del RDC / TMD (Research Diagnostic Criteria for Temporomandibular Disorders). Los datos recolectados fueron evaluados usando el programa SPSS usando las pruebas de Chi cuadrado exacto de Wilcoxon, Kruskal Wallis y Fisher. Resultados: PO y F mostraron un comportamiento similar en el patrón de apertura, manteniendo el mismo patrón que el paciente durante el tiempo de tratamiento (P = 0,003). En la amplitud máxima de apertura sin asistencia, ambas terapias mostraron una mejora significativa en la ganancia de amplitud en el tiempo inicial y después de un mes de terapia (P = 0,002), manteniendo la amplitud hasta el tercer mes, sin presentar ganancia significativa. Conclusión: PO y F mostraron un comportamiento similar para el tratamiento de TMD al analizar el patrón de apertura y la amplitud media máxima de apertura sin asistencia.Objetivo: Avaliar a amplitude e o padrão de abertura bucal em pacientes diagnosticados com Disfunção Temporomandibular (DTM) que foram submetidos à tratamento com Placa Oclusal (PO) e Fisioterapia (F) após 1 e 3 meses de tratamento. Metodologia: Foram avaliados o padrão de abertura bucal e a amplitude de abertura bucal de 48 pacientes submetidos a um ensaio clínico randomizado e divididos em 2 grupos de tratamento: placa oclusal (PO; N= 23) e fisioterapia (F; N = 25). Todos os pacientes foram diagnosticados com DTM de acordo com o eixo 1 do RDC/TMD (Research Diagnostic Criteria for Temporomandibular Disorders). Os dados colhidos foram avaliados através do programa SPSS usando os testes Wilcoxon, Kruskal Wallis e Qui Quadrado Exato de Fisher. Resultados: A PO e F apresentaram desempenho semelhante no padrão de abertura, mantendo o mesmo padrão do paciente ao longo do tempo de tratamento (P = 0.003). Na amplitude máxima de abertura sem auxílio ambas as terapias apresentaram melhora significativa de ganho de amplitude no tempo inicial e após um mês de terapia (P = 0.002), mantendo a amplitude até o terceiro mês, sem apresentar ganho significativo.
