Conservative treatment of mandibular keratocyst - 1 year follow-up

Resumo: The keratocyst is a rare development cyst with high destructive power and can reach large dimensions and a high rate of recurrence. Most of this injury affects men aged 20 to 40 years, affecting the posterior region of the mandible. It can be asymptomatic when in small dimensions even with slow growth, the diagnosis can often be observed in radiographic control exams that are used in our daily clinical practice. The purpose of this case report was to demonstrate the conservative treatment of Odontogenic Keratocyst (OK) in a six-year-old child. The patient underwent an image exam (panoramic radiography) and a surgical procedure with the result of a pathological anatomy. It is important to inform that the objective was achieved with the surgical, but it is worth mentioning that the discomfort caused by the device and the difficulty of cleaning the patient was present. It is noted that the follow-up of this study was for one year and we believe that the patient should continue to attend preventive consultations, due to the high power of recurrence of this pathology. We conclude that decompression was a method used as a conservative treatment and this treatment option continues to be considered successful and much less invasive among the possibilities presented for this type of pathology.

La mayor parte de esta lesión afecta a hombres de 20 a 40 años, afectando la región posterior de la mandíbula. Puede ser asintomático cuando en pequeñas dimensiones, incluso con un crecimiento lento, el diagnóstico a menudo se puede observar en los exámenes de control radiográfico que se utilizan en nuestra práctica clínica diaria. El propósito de este informe de caso fue demostrar el tratamiento conservador del queratoquiste odontogénico (OQ) en un niño de seis años. El paciente se sometió a un examen de imagen (radiografía panorámica) y un procedimiento quirúrgico con el resultado de una anatomía patológica. Es importante informar que el objetivo se logró con la maniobra quirúrgica, pero vale la pena mencionar que la molestia causada por el dispositivo y la dificultad de limpiar al paciente estaban presentes. Se observa que el seguimiento de este estudio fue de un año y creemos que el paciente debe continuar asistiendo a consultas preventivas, debido al alto poder de recurrencia de esta patología. Llegamos a la conclusión de que la descompresión era un método utilizado como tratamiento conservador y esta opción de tratamiento continúa considerándose exitosa y mucho menos invasiva entre las posibilidades presentadas para este tipo de patología.O queratocisto é um cisto de desenvolvimento raro de alto poder de destruição podendo atingir grandes dimensões e de alto índice de recidiva. Em sua grande maioria essa lesão acomete homens na faixa de 20 a 40 anos acometendo a região posterior de mandíbula. Pode ser assintomático quando em pequenas dimensões mesmo apresentando um crescimento lento, muitas vezes, o diagnóstico pode ser amplamente observado em exames radiográficos de controle que são usados em nosso dia a dia clínico. O objetivo deste relato de caso foi demonstrar o tratamento conservador do Queratocisto Odontogênico (QO) em uma criança de seis anos de idade. O paciente foi submetido ao exame de imagem (radiografia panorâmica) e ao procedimento cirúrgico com resultado de anátomo patológico. É importante informar que o objetivo foi alcançado com a manobra cirúrgica, porém vale ressaltar que o desconforto pelo dispositivo e a dificuldade de higienização do paciente se fez presente. Nota-se que o acompanhamento deste estudo foi de um ano e é recomendado que o paciente deva permanecer indo as consultas preventivas, devido ao alto poder de recorrência desta patologia.
