Diagnosis and treatment of elements affected by molar incisor hypomineralization: a clinical case report

Resumo: Molar Incisor Hypomineralization (MIH) is characterized as a defect in enamel development. Elements that have this systemic alteration clinically present with demarcated white, yellow or brownish opacities. Its etiology remains inconclusive and its cause is considered multifactorial, making it a challenge in pediatric dentistry offices. The article aims to report a clinical case of severe HMI (molar incisor hypomineralization) in an 8-year-old child. For its development, an exploratory search was carried out on Google academic, Pubmed, Scielo of the last ten years and a specific book on the exposed topic. Patient J.L.S.L male, attended the UNITPAC children's clinic with the mother's main complaint 'my son's teeth are very yellow' and difficulty brushing due to tooth sensitivity. Clinically, marked opacities were observed in the dental enamel, with a color ranging from white to yellowish tones, characteristics of HMI, on the buccal surfaces of the maxillary incisors and molars, and on the occlusal surfaces of the 1st mandibular molars with loss of tooth structure. No element showed fistula or any other characteristic sign of pulp involvement. The child was then diagnosed with severe HMI and the proposed treatment required multiple sessions of topical application of varnish, ART restoration in the elements with cavity and use of resin sealant, followed up for 6 months. All in all, we can conclude with the importance of a detailed anamnesis so that there is an early diagnosis, considering the most conservative treatments possible and improving the sensitivity framework.

Los elementos que presentan esta alteración sistémica se presentan clínicamente con opacidades delimitadas de color blanco, amarillo o pardusco. Su etiología sigue siendo inconclusa y su causa se considera multifactorial, lo que la convierte en un desafío en los consultorios de odontopediatría. El artículo tiene como objetivo reportar un caso clínico de HMI (hipomineralización incisivo molar) severa en un niño de 8 años. Para su desarrollo se realizó una búsqueda exploratoria en Google académico, Pubmed, Scielo de los últimos diez años y un libro específico sobre el tema expuesto. Paciente J.L.S.L masculino, acudió a la clínica infantil de UNITPAC con principal queja de la madre 'mi hijo tiene los dientes muy amarillos' y dificultad para el cepillado por sensibilidad dental. Clínicamente se observaron marcadas opacidades en el esmalte dentario, con una coloración que va del blanco a tonos amarillentos, características de HMI, en las caras bucales de los incisivos y molares maxilares, y en las caras oclusales de los primeros molares mandibulares con pérdida de dientes estructura. Ningún elemento presentaba fístula ni ningún otro signo característico de afectación pulpar. Luego, el niño fue diagnosticado con HMI grave y el tratamiento propuesto requirió múltiples sesiones de aplicación tópica de barniz, restauración ART en los elementos de la cavidad y uso de sellador de resina, con un seguimiento de 6 meses. Con todo, podemos concluir con la importancia de una anamnesis detallada para que exista un diagnóstico precoz, considerando los tratamientos más conservadores posibles y mejorando el marco de sensibilidadA HMI é caracterizada como um defeito no desenvolvimento do esmalte. Os elementos que possuem essa alteração sistêmica apresentam-se clinicamente opacidade demarcadas de coloração branca, amarela ou amarronzada. A sua etiologia continua inconclusiva e sua causa é considerada multifatorial, tornando um desafio nos consultórios de odontopediatria. O artigo tem como objetivo de relatar um caso clínico sobre HMI (hipomineralização molar incisivo) severa em uma criança de 8 anos. Para o seu desenvolvimento foi realizado uma busca exploratória no Google acadêmico, Pubmed, Scielo dos últimos dez anos e livro específico sobre o tema exposto. Paciente J.L.S.L sexo masculino, compareceu à clínica infantil do UNITPAC com queixa principal da mãe ‘os dentes do meu filho estão muito amarelados’ e, dificuldade de escovação devido a sensibilidade dentária. Clinicamente foram constatadas opacidade demarcadas no esmalte dentário, de coloração variando de branco a tons de amarelados, características de HMI, nas superfícies vestibulares dos incisivos e molares superiores, e nas oclusais dos 1° molares inferiores com perda de estrutura dentária. Nenhum elemento apresentou fístula ou qualquer outro sinal característico de envolvimento pulpar. A criança foi então diagnosticada com HMI severa e o tratamento proposto necessitou de múltiplas sessões de aplicação tópica de verniz, restauração ART nos elementos com cavidade e uso de selante resinoso, acompanhamento durante 6 meses.
