Breastfeeding of premature infants: integrative review from 2015 to 2020

Resumo: | The context of breastfeeding experienced by mothers of premature newborns is the main issue of this article, considering the needs of growth, neurological development, clinical stability and thermoregulation control for the initiation of early stimulation of breastfeeding. On the other hand, we have the sociocultural condition of the mother who needs support and guidance from the multidisciplinary team to cross the situations that come from the hospitalization of her child in the neonatal unit. In this way, it is essential to provide health education on breastfeeding and daily care for premature infants. This article lists breastfeeding practices in neonatal units from 2015 to 2020. Integrative review of the literature conducted in the Virtual Health Library and MEDLINE, LILACS and BDENF databases using the descriptors in Health Sciences (DeCS): breastfeeding, premature newborn, neonatal ICU; in six phases: preparation of the review question, search of articles according to inclusion and exclusion criteria, data collection, critical analysis of selected articles, discussion of results and presentation of the review. Selected 148 articles, of these 20 articles composed the review. Level of evidence of the articles included: 05 meta-analyses - Level I, 07 cross-sectional studies - Level IV, 06 descriptive qualitative studies and 02 integrative reviews - Level V. The evidence described in the analyzed articles corroborates that breastfeeding combined with available health technologies increases the survival of premature infants. Policies and strategies for strengthening breastfeeding of premature infants should be implemented in public health policies. |

Por otro lado, tenemos la condición sociocultural de la madre que necesita apoyo y orientación del equipo multidisciplinario para atravesar las situaciones que se derivan de la hospitalización de su hijo en la unidad neonatal. De esta manera, es fundamental proporcionar educación sanitaria sobre la lactancia materna y el cuidado diario de los bebés prematuros. Este artículo enumera las prácticas de lactancia materna en unidades neonatales de 2015 a 2020. Revisión integradora de la literatura realizada en la Biblioteca Virtual en Salud y en las bases de datos MEDLINE, LILACS y BDENF utilizando los descriptores en Ciencias de la Salud (DeCS): lactancia materna, recién nacido prematuro, UCI neonatal; en seis etapas: preparación de la pregunta de la revisión, búsqueda de artículos según criterios de inclusión y exclusión, recolección de datos, análisis crítico de artículos seleccionados, discusión de resultados y presentación de la revisión. Seleccionados 148 artículos, de estos 20 artículos compusieron la revisión. Grado de evidencia de los artículos incluidos: 05 metanálisis - Nivel I, 07 estudios transversales - Nivel IV, 06 estudios cualitativos descriptivos y 02 revisiones integradoras - Nivel V. La evidencia descrita en los artículos analizados corrobora que la lactancia materna combinada con las tecnologías de salud disponibles aumenta la supervivencia de los bebés prematuros. Las políticas y estrategias para fortalecer la lactancia materna de los bebés prematuros deben implementarse en las políticas de salud pública.O contexto do aleitamento materno vivenciado por mães de recém-nascidos prematuros constitui a questão norteadora deste artigo, considerando as necessidades de crescimento, desenvolvimento neurológico, estabilidade clínica e controle da termorregulação para início da estimulação precoce da amamentação. Em contrapartida temos a condição sociocultural da mãe que necesita de apoio e orientações da equipe multiprofissional para travessia das situações advindas da internação de seu filho na unidade neonatal. Nesse caminho é indispensável propiciar a educação em saúde sobre a amamentação e os cuidados diários com o prematuro. Revisão integrativa da literatura sobre aleitamento materno em unidade neonatal no período de 2015 a 2020 realizada na Biblioteca Virtual em Saúde e bases de dados MEDLINE, LILACS e BDENF usandos os descritores em Ciências da Saúde (DeCS): aleitamento materno, recém-nascido prematuro, UTI neonatal; em seis etapas: elaboração da questão de revisão, busca dos artigos de acordo com critérios de inclusão e exclusão, coleta de dados, análise crítica dos artigos selecionados, discussão dos resultados e apresentação da revisão. Selecionados 148 artigos, destes 20 artigos compuseram a revisão. Nível de evidência dos artigos incluídos: 05 meta-análises - Nível I, 07 estudos transversais - Nível IV, 06 estudos qualitativos descritivos e 02 revisões integrativas - Nível V. As evidências descritas nos artigos analisados corroboram que o aleitamento materno aliado as tecnologias da saúde disponíveis aumentam a sobrevida dos prematuros.
